Terms of Participation
1. Choose your ad size/design option and indicate whether you will provide a new ad or reuse last year’s ad.
2. Please submit payment electronically or mail payment along with your ad submission.
3. Ads attached to this form or submitted by email must be in PNG or JPEG format.
4. If your ad is not attached to this form, please email all ad designs to kaitlynchess@gmail.com.
5. Ad designs are due by June 15, 2026. All ads will be submitted to the printer by July 15, 2026.
6. Ads promoting drugs, alcohol, tobacco, or inappropriate content will not be permitted.

By signing below, the customer acknowledges that they have read, understand, and agree to abide by the Policies of the Onalaska Apple Harvest Festival, including all terms and conditions outlined therein.

The customer agrees to pay the amount indicated on this form based on the selected ad size. Payment must be received before the ad will be included in the AHF Event Guide. The customer is responsible for submitting their ad by the June 15, 2026 deadline.

All proceeds and donations support the operation of the Onalaska Alliance, a 501(c)(3) nonprofit organization (EIN: 26-4577681).

Customers will receive a confirmation email upon completion of this form and should check their spam or junk folder if necessary. The Event Guide will be printed by mid-August and distributed through local businesses.
