  • Documentation & Payment Policy

  • Purpose
    To ensure accuracy, compliance, and continuity of care for all clients served by First-Rate Caregivers Health. Timely and complete charting is essential for clinical quality, legal protection, and proper billing.
  • Policy Statement
    All caregivers are required to fully complete and submit documentation for each visit before payment can be issued. Any visit that is not charted in its entirety will not be eligible for payment.
  • Documentation Requirements
    To qualify for payment, all of the following must be completed:
    • Visit note documented in full (no missing sections).
    • Clinical details accurately recorded, including vital information relevant to the patient’s condition.
    • Tasks/interventions completed and checked off, with explanations if any tasks were missed.
    • Accurate time in/time out documentation.
    • Required signatures obtained when applicable.
    • Submission within the company’s required timeframe.
  • Review and Payroll Approval
    Supervisors and administrative staff will review all documentation before approving payroll. Payment will only be released when charting meets company standards.
  • Consequences of Non-Compliance
    Visits with incomplete documentation will not be paid until the documentation is corrected and verified. Repeated failure to document properly may lead to disciplinary action, up to termination.
