• Employee Resignation Policy

    Formally submit your resignation or notice per company policy. Please complete all required fields and review the policy acknowledgment.
  • To support continuity of patient care and ensure adequate staffing coverage, First-Rate Caregivers Health requires all employees to provide proper notice when resigning. This policy explains resignation expectations and how failure to comply may affect final pay.

    1.     Requirement for Resignation Notice Employees must:

    ·        Submit a written two (2) week notice of resignation.

    ·        Complete all scheduled shifts during the notice period unless excused by management.

     

    2.     Impact on Final Pay if Notice Is Not Given

    If an employee resigns without providing a two-week written notice or fails to complete the notice period:

    ·        The company may adjust the final pay rate to minimum wage for hours worked during the final pay period.

    ·        Pay will not be reduced below federal or state minimum wage.

     

    3.     Conditions Where Pay Will NOT Be Adjusted

    ·        Full notice is provided and completed.

    ·        Separation is due to medical emergency or legally protected circumstances.

    ·        The employee is terminated by the company.

     

    4.     Acknowledgment & Consent

    By signing, the employee acknowledges and agrees to the terms in this policy.
