The warranty for all prosthetic work is 3 years. Patheodent will repair or replace without charge to patient any covered treatment that fails due to faulty materials.
Treatments eligible for coverage:
The following types of treatments are eligible for coverage under the limited 3-year warranty agreement: Crowns, bridges, inlays, onlays, abutments and implant crowns.
How to obtain care and service: In the event of a failure of a covered treatment, the patient must notify Patheodent of the failure within a reasonable time after discovery (not to exceed 7 days) and schedule and attend to an appointment with Patheodent to take or schedule corrective action. Patient's failure to timely notify Patheodent of a failure is grounds for voiding coverage.
Term: Coverage of Covered treatments begins on the treatment date
What is not covered: This limited warranty agreement does not cover: Trauma, pre-existing conditions, Bruxism, Cosmetic discoloration, failure of a covered treatment due to: failure of patient to comply with his or her responsibilities, other treatments, medical conditions resulting from substance abuse, medical conditions that cause damage to oral structures including the supporting bone around implants, loss or a removable prosthetic or appliance.
Patient responsibilities: Patient is responsible for routine dental care and proper oral and dental hygiene. Failure to receive routine dental care and proper dental hygiene will void coverage. Smoking, drug use and other abusive habits will also void coverage. Patient is responsible for providing proof of routine dental care. This limited warranty is not an insurance policy.
