  • I hereby authorize Darul Uloom Michigan to initiate monthly fee payments from my designated account. This authorization will remain in effect until I provide written notice to Darul Uloom Michigan requesting any changes. I understand that withdrawal of my child from Darul Uloom Michigan is not permitted during the middle of the academic year. If I choose to withdraw my child, I am responsible for paying the monthly fees through the end of the academic year. If the direct deposit is canceled by me or by my bank, I understand that I will be responsible for any applicable cancellation fees.
