In compliance with the Personal Information Privacy Act, Valley Christian School Online requires the consent of parent(s) or guardian(s) to collect, store and utilize personal information. I/we consent to having VCOS collect personal information that may include: student identification information, birth certificate, legal guardianship papers, court orders it’s applicable, parents work numbers and email addresses, emergency contact name and number, doctor's name and number, health insurance number, behavioural, academic and health information, most recent report card, and any similar information needed for registration. I further consent to the use and disclosure of information contained in this form and otherwise collected by or on behalf of VCS for the purpose of establishing maintaining and terminating the students or parents relationship with VCS for the additional purpose of the purposes identified when or before personal information is collected and otherwise provided in vicious is personal information privacy policy a copy of which is available on request I also consent to the collection use and disclosure of such personal information by and to agents contractors and service providers of VCOS. This information is required to register your child in Valley Christian School Online and assist school authority in making an informed decision as to your child's suitability and appropriate placement in the school. Promotional Consent: I/We Consent to having photographs and videos and work samples of my child used by VCSO in newsletters, Facebook, Instagram, website and other school promotional material as well as full names used in the yearbook. School Directory Consent: I/We Consent to having our family information names phone numbers address included in the internal school phone directory for the purpose of class list. Valley Christian School acknowledges that there will be no disclosure of personal information to unauthorized personnel or third parties or not directly involved in school management (care, supervision or instruction) of your child at the school unless written authorization from a parent or legal guardian is provided to the school.
