  • Athlete Participation Acknowledgment & Release

    By submitting this form, I acknowledge that the Sports Prodigy Long-Term Development Program involves non-equipment, limited-contact activities such as drills, instruction, and conditioning. I confirm that the athlete is physically able to participate and understand that participation still involves risk of injury, which I voluntarily assume.

    I release the Sports Prodigy Long-Term Development Program organizers, coaches, staff, and facility from any liability for injury, illness, or loss arising from participation, except in cases of gross negligence.

    I authorize the Sports Prodigy Long-Term Development Program staff to seek emergency medical treatment for the athlete if necessary and understand that I am responsible for any related medical expenses.

    I grant permission for photos and videos taken during the Sports Prodigy Long-Term Development Program to be used for promotional or educational purposes.
