I am parent/legal guardian and give permission for my child to participate in TippyToes & Cute-icles Nail academy summer nail camp.

I understand that this program includes beginner nail activities, nail art projects, use of nail products, and supervised hands on learning.

I acknowledged that while safety and sanitation procedures will be followed, participation may involve minor risks, including allergic reactions, skin irritation, or accidental injury.

I release and hold harmless TippyToes & Cute-icles Nail Academy, its owner, instructors, assistants, staff, and affiliates from liability for injuries, damages, losses, claims, or expenses resulting from participation in camp activities.

I authorize emergency medical treatment if necessary in the event I cannot be reached immediately.

MEDIA RELEASE:I also give permission for photographs and/or videos taken during camp activities to be used for promotional, educational, and marketing purposes unless I submit a written request declining media use.

PAYMENT POLICY:I understand the $50 non-refundable deposit is required to secure my child’s spot. Remaining balances must be paid prior to the start of camp.
