2026 Graduates Form

Graduate Submission Form

We are excited to celebrate the accomplishments of our 2026 graduates. Please complete the brief form to collect graduate details and upload photos in preparation for our official celebration during the worship service on June 13th, 2026. Please submit your submission by May 30, 2026, to be considered for recognition and the award.

*Students must meet the following eligibility requirements:
- Students must be attending a new school or have participated in an actual graduation.
- The student must be a church member, or the student's parent or guardian must be.
- Students who attend Germantown but whose parent or guardian is not a member must participate actively in youth activities (NeXT Youth Ministry, Children's Choir, Pathfinders, or Connect Team, etc).
- Adult students must be members of Germantown or actively working toward membership (i.e., bible study, member transfer, member by profession of faith, etc.).
