I understand that I am attending the MDC Small Talk Conversation Club as a non-member.

I understand that I am responsible for my own safety and personal belongings during club activities.

I understand that I attend club activities at my own risk.

I agree that the MDC Small Talk Conversation Club, its student leaders, and participants are not responsible for any accidents, injuries, loss, or damages that may occur during my participation.

By signing this form, I waive and release any claims against the MDC Small Talk Conversation Club, its student leaders, and participants related to any accidents, injuries, loss, or damages that may occur during my participation.

Entiendo que estoy asistiendo al MDC Small Talk Conversation Club como no miembro.

Entiendo que soy responsable de mi propia seguridad y de mis pertenencias personales durante las actividades del club.

Entiendo que asisto a las actividades del club bajo mi propio riesgo.

Acepto que el MDC Small Talk Conversation Club, sus líderes estudiantiles y participantes no son responsables por accidentes, lesiones, pérdidas o daños que puedan ocurrir durante mi participación.

Al firmar este formulario, renuncio y libero de cualquier reclamo al MDC Small Talk Conversation Club, a sus líderes estudiantiles y a sus participantes relacionado con cualquier accidente, lesión, pérdida o daño que pueda ocurrir durante mi participación.

This waiver and consent form is valid for all future MDC Small Talk Conversation Club activities that I attend as a non-member.

I understand that I do not need to complete this form again each time I attend, unless the club asks me to update it.

Este formulario de exención y consentimiento es válido para todas las futuras actividades del MDC Small Talk Conversation Club a las que asista como no miembro.

Entiendo que no necesito completar este formulario cada vez que asista, a menos que el club me pida actualizarlo.
