Statement of Agreement:
I, the undersigned, as the guardian/parent of the minor child listed above, understand that BC Choral Federation and the instructors of Choral Stage thereof are not liable for personal injuries or loss of, or damage to, personal property. I understand that participating in movement classes involves the possibility of physical injury. I exempt, release, indemnify and hold harmless the BC Choral Federation, its officers, its employees, and the Choral Stage faculty thereof, from any and all liability claims, demands or causes of action whatsoever from any damage, loss or physical injury to myself or my participating minor child. By signing below, I am acknowledging that I have read and agreed to be bound by all the rules and regulations as stated on this form.
Photo/Video Release:
I hereby consent to and authorize the use and reproduction by Choral Stage of any and all photography and/or videography that has been taken of me or my minor child during the Choral Stage Camp (July 6-17, 2026) for any purpose. I understand that neither myself nor my minor child will receive any monetary compensation now or in the future for our appearance(s) therein. I understand that the BC Choral Federation reserves the right to use the images and/or video for online, educational and promotional purposes, including social media sites of its choosing. By signing below, I hereby acknowledge that I am the parent/guardian of the minor child listed above and have read and understood the terms of this release.
