  • Sweet Jubilee LLC DBA Jubilee's Playhouse – Liability Waiver & Release of Claims

     

    IMPORTANT – PLEASE READ CAREFULLY. This waiver must be signed by a parent or legal guardian before any child may participate in activities at Sweet Jubilee LLC DBA Jubilee's Playhouse

     

    Assumption of Risk: I understand that participation in activities at Sweet Jubilee LLC DBA Jubilee's Playhouse, including but not limited to: use of the indoor playground, playhouses, climbing structures, slides, foam pits, candy features, and general play equipment, involves inherent risks of injury, illness, or other potential hazards. I voluntarily assume all risks, both known and unknown, associated with such participation.


    Waiver & Release: In consideration for being allowed to use the facilities and participate in events at Sweet Jubilee LLC DBA Jubilee's Playhouse, I, for myself and on behalf of my child(ren), heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless Sweet Jubilee LLC DBA Jubilee's Playhouse, its owners, agents, employees, contractors, and volunteers from any and all liability, claims, demands, losses, or damages arising from participation, including but not limited to personal injury, property damage, illness (including communicable diseases), or accidental death.
    Supervision & Responsibility: I understand that it is my responsibility to supervise my child(ren) at all times unless otherwise noted in writing by staff. I agree to follow all posted rules and directions given by staff. I understand that failure to do so may result in removal from the premises.
    Health Acknowledgement: I confirm that my child(ren) are in good health and not exhibiting any signs of illness, including but not limited to fever, rash, vomiting, or communicable disease. I agree not to bring any child(ren) showing symptoms of illness into the facility.
    Photo & Media Release: I grant permission for photos or videos taken during our visit to be used for promotional purposes by Sweet Jubilee LLC DBA Jubilee's Playhouse. No names or identifying information will be shared.
    Agreement to Terms: I have read and fully understand this waiver. I certify that I am the parent or legal guardian of the child(ren) named below and that I am legally authorized to sign this waiver on their behalf. I agree to be bound by its terms.
