MMCC Shakespeare Camp Participation Waiver
By signing this form, I agree that I am the child's legal guardian and authorize participation in the MMCC Shakespeare Camp.
I understand that my child's participation in MMCC Summer Day Camp involves inherent risks. This waiver is intended to release Madison-Morgan Cultural Center and the Hall on Foster and its staff from any liability for injuries or damages that may occur during my child's participation.
I hereby release, discharge, and waive any and all claims against Madison-Morgan Cultural Center (MMCC) including all officers, members of the board of directors, employees of MMCC, contract instructors and volunteers for any and all injuries, damages, or losses suffered by my child while participating in camp activities.This includes, but is not limited to, any claims arising from negligence or the conduct of any person in connection with the camp.
I assume all risks and hazards incidental to the conduct of activities to and from MMCC. I further agree to indemnify and hold harmless MMCC from any and all claims, demands, liabilities, and costs, including attorney's fees, arising out of my child's participation.
I understand health, or accident insurance that would cover my medical, hospital or related expenses in the event of injury in this activity is my responsibility, l understand MMCC strongly recommends that if I do not have sufficient insurance to cover such incidents that I should take the necessary actions to obtain insurance participating in any class or workshop.
I have read and understand the MMCC Shakespear Camp waiver.
