2026 - 2027 Youth Competitive Team Registration Form

This form will need to be completed for EACH dancer you are registering. If you are registering multiple dancers from your family, each dancer requires their own registration form to be completed.
Thank you for your interest in the 2026–2027 Level Up Dance Complex Competitive Program! We are excited to continue building a positive, challenging, and inspiring season for our dancers and families.

This registration form is designed to help us better understand your dancer’s interests, goals, and availability for the upcoming competitive season. Please note that submission of this form does not guarantee placement, routine assignments, or participation in specific competitive opportunities.

Following Placement/Contract Day, families will receive finalized team placements, choreography decisions, and competitive assignments via the email address provided in this registration. Prior to the official start of the season, families will also receive a detailed estimated cost breakdown for the full competitive season, including tuition, competition fees, costumes, conventions, and any applicable performance-related expenses. We believe in transparency and want all families to feel informed and prepared before committing to the season ahead.

Competitive Interest Form
Please note that the selections below are intended to express interest only and do not guarantee placement, participation, or assignment in any solo, duo, trio, small group, or additional competitive opportunity. Final decisions regarding choreography assignments, number of routines, groupings, and participation are made solely at the discretion of Level Up Dance Complex staff based on factors including, but not limited to, skill level, technical ability, maturity, rehearsal attendance, team needs, dancer compatibility, and overall program structure for the season.

All additional opportunities include up to 4 extra hours of rehearsals per month. Title entries receive additional rehearsal slots per month.
