  • HOLD HARMLESS / LIABILITY WAIVER

     I, the undersigned parent/legal guardian of the above-named participant, understand that participation in cheerleading, dance, tumbling, stunting, conditioning, camps, clinics, showcases, and related activities through TNT Spirit Explosion Cheer Dance & Tumble LLC and its affiliates involves physical activity and potential risk of injury.

     I voluntarily allow my child to participate and acknowledge that injuries may occur, including but not limited to sprains, fractures, concussions, permanent injury, or other unforeseen accidents.

     In consideration of participation in TNT Spirit Explosion Cheer Dance & Tumble LLC activities, I agree to:

     Release, waive, and discharge TNT Spirit Explosion Cheer Dance & Tumble LLC, Coach Nikki, staff members, volunteers, affiliates, sponsors, facility owners, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of participation in any program or activity.
    Hold harmless and indemnify TNT Spirit Explosion Cheer Dance & Tumble LLC and its affiliates from any loss, damage, injury, or expense that may occur.
    Accept full responsibility for any medical expenses incurred due to injury or illness during participation.
    Confirm that my child is physically able to participate in activities and has no medical condition that would prevent safe participation.

     I understand that although safety precautions are taken, accidents may still happen.

     

    MEDICAL AUTHORIZATION

    In the event of an emergency, I authorize TNT Spirit Explosion Cheer Dance & Tumble LLC staff to obtain emergency medical treatment for my child if I cannot be reached immediately.

     

    IMAGE / VIDEO RELEASE AGREEMENT

    I grant permission to TNT Spirit Explosion Cheer Dance & Tumble LLC and its affiliates to photograph, videotape, and record my child during practices, classes, camps, competitions, performances, events, and related programs.

    I understand and agree that these images/videos may be used for:

    Social media posts
    Promotional flyers
    Marketing materials
    Website content
    Advertisements
    Livestreams
    Print materials
    Fundraising campaigns
    Team and gym promotions

    I understand that no compensation will be provided for the use of such images or videos.

    I also release TNT Spirit Explosion Cheer Dance & Tumble LLC and its affiliates from any claims regarding the use of these materials.
