    By providing this information you are authorizing, a representative of GCAO to contact your veterinarian in regards to the vaccination & health status of your current animals. This information will only be accessible by authorized GCAO representatives to aid in proper placement of foster animals.

  • By submitting this application I affirm that the facts set forth above are true and complete to the best of my knowledge. I understand that if I am accepted as a foster, I am not the rightful owner of the dog(s) or cat(s) and any medical and/or rehoming decisions will be made by GCAO. I understand that any false statements, ommisions or other misrepresentaions made by me on this application may result in the immediate termination of the foster agreement/contract at the sole discrection of GCAO. If GCAO terminates the agreement/contract I agree to make the foster animal immediatley available for pickup by a GCAO representative. 

    I understand and aree that Grant County Animal Outreach, its officers, directors, staff, and volunteers are held harless from and against any and all claimsand damages of every kind for injury to any person or performance of the above named volunteer (foster) under the agreement, including claims and damages arising in whole or in part from the negligence of Grant County Animal Outreach. 

    I understand and agree that Grant County Animal Outreach (GCAO) cannot guarantee the temperament, behavior, health, or actions of any foster animal, whether known or unknown, including but not limited to interactions with people, children, other animals, or property. I acknowledge that animals may react unpredictably in new environments and that by voluntarily fostering an animal, I assume all risks associated with the care and housing of the animal. I further understand and agree that GCAO, its officers, directors, staff, and volunteers shall not be held liable for any injury, illness, damages, loss, or death to persons, animals, or property that may occur during the foster period. I accept full responsibility for the foster animal while in my care, including any damages or incidents caused by the animal. I also understand that all placement and rehoming decisions remain solely at the discretion of GCAO, and that GCAO will place animals to the best of its abilities and judgment.

    I understand and agree that my electronic signature shall have the same legal effect and enforceability as an original handwritten signature. By electronically signing this agreement, I acknowledge that I have carefully read, understood, and voluntarily agreed to all terms, conditions, policies, and responsibilities outlined herein. I further acknowledge that this electronic agreement is legally binding and enforceable under applicable state and federal laws governing electronic signatures and records.
