In consideration for being allowed to participate in this Activity/Program, I release from liability and waive my right to sue Global Islamic Centre, their employees, officers, volunteers and agents (collectively “Global Islamic Centre”) from any and all claims, including claims of the Global Islamic Centre’s negligence, resulting in any physical injury, illness (including death) or economic loss that my child/children may suffer or which may result from their participation in this program, travel to and from the program, or any events incidental to this program. This includes any risks associated with transportation provided by school bus to and from field trip locations. I agree that Global Islamic Centre may use photographs of my children for any lawful purpose, including publicity, illustration, advertising, and Web content.
