Youth Participant Liability Waiver, Release, and Media Consent

Please review the following terms of participation.
Parent / Legal Guardian Agreement

I am the parent or legal guardian of the child named above (“Participant”). I acknowledge that my child is voluntarily participating in a youth skills camp at Glen Oaks Community College (“GOCC”).

I understand that participation in athletic or recreational activities involves inherent risks, including but not limited to slips, falls, collisions, or other injuries. I knowingly and voluntarily assume all such risks on behalf of my child.

Release of Liability

To the fullest extent permitted by law, I hereby release, waive, and discharge Glen Oaks Community College, its Board of Trustees, officers, employees, volunteers, agents, and representatives from any and all claims, demands, actions, or causes of action arising out of or related to my child’s participation in this event, including injury, illness, or property loss, whether caused by negligence or otherwise.

I understand that Glen Oaks Community College does not provide medical or accident insurance coverage for participants.

Medical Authorization

In the event of an emergency, I authorize Glen Oaks Community College staff or representatives to obtain medical treatment for my child if I cannot be reached. I understand that I am financially responsible for any medical care provided.
