  • I understand that I am not allowed to volunteer at the Boys & Girls Clubs of Harrison – Crawford Counties prior to completing orientation and approval of formal background check. I authorize the Boys & Girls Clubs of Harrison – Crawford Counties to investigate all statements in this application. I understand that a background check will be conducted at the expense of the organization which may include a verification of my motor vehicle driving record, criminal and civil records, prior employment (including contacting prior employers), education (degree, GPA, and attendance) and/or other public record information that may be deemed necessary by the Boys & Girls Club or its designees. I authorize the release of such information as may be necessary to verify the information I have provided. I release and hold harmless from liability any individual or entity requesting or supplying information with respect to my application for volunteer service at the Boys & Girls Club. This application is for volunteer (i.e., unpaid) positions at the Club. I authorize for my picture, video, or any reproduction thereof (while engaged in Club-related activities) to be used for publicity/fundraising purposes.
