1. I understand that Waxahachie Snipers Summer Basketball Camp is a basketball skills and player development camp held Monday through Thursday from 8:00 AM to 3:00 PM.

2. I understand that basketball and athletic activity involve risk of injury, including but not limited to sprains, strains, falls, collisions, heat-related illness, and other injuries.

3. I give permission for my child to participate in all camp activities unless restrictions are listed in the medical section of this form.

4. I certify that the medical and emergency information provided is accurate to the best of my knowledge.

5. I authorize Waxahachie Snipers Basketball Club, its coaches, staff, volunteers, or facility partners to seek emergency medical care for my child if I cannot be reached in an emergency.

6. I understand that my child must bring lunch, a water bottle, and appropriate basketball shoes each day unless otherwise instructed.

7. I understand that players are expected to show respect to coaches, staff, volunteers, other players, the church facility, and all camp equipment.

8. I understand that serious or repeated misconduct, fighting, bullying, disrespect, unsafe behavior, or refusal to follow instructions may result in removal from camp without refund.

9. I understand that registration is not complete until payment is submitted.

10. I understand that only one discount may be used per camper and discounts may not be combined.

11. I understand that if the wrong registration type is selected, the remaining balance must be paid before my child may attend camp.

12. I understand that drop-off and pick-up procedures must be followed for camper safety.
