By completing this form, you are requesting a refund of course fees in part or in full.

Please provide all relevant information and supporting documentation to assist with the review of your request.

Refund applications are assessed on a case-by-case basis and will be processed within four (4) weeks of receiving the completed request. Any approved refund will only be paid to the person or organisation that entered into the agreement with CSI.

All refunds are paid electronically. Cash refunds will not be provided.
