Please note that each participant must complete their own booking form using their own personal details. The information provided on this form is used to confirm the participant’s booking, ensure we have the correct contact details, and help our team prepare for the guided shore snorkelling experience safely and efficiently.

Each participant must have their own email address, as all booking confirmations, important snorkelling information, indemnity details, and any updates regarding weather, sea conditions, meeting times, or changes to the schedule will be sent directly to that email address.

Multiple participants cannot be booked under one person’s details or email address, as this may cause delays with communication, check-in, and activity administration.

For participants under the age of 18, the booking form must be completed by a parent or legal guardian. Only information relevant to the participant’s booking, safety, emergency contact details, and indemnity requirements should be provided. Please do not submit unnecessary or unrelated personal information.

Before completing this booking form, please note that guided shore snorkelling is weather and condition dependent. The snorkelling site, meeting time, and final arrangements may be adjusted closer to the time to ensure the safest and best possible experience.

Your booking will only be confirmed once the Dive School has checked availability and confirmed your selected snorkelling date.

As part of this booking form, you will be required to complete the medical questionnaire. Please note that this medical questionnaire will also need to be completed again once your PADI eLearning materials have been assigned, as PADI requires this information to be verified through their system as well. The questionnaire on this form helps us check, before confirming your booking, whether you may need medical clearance from a physician before taking part in any scuba diving activities.

Please read each medical question carefully and answer honestly. Certain medical answers may require a physician’s approval before you can continue with the course. If this is only identified after your eLearning has already been assigned, it may result in your course being postponed or cancelled, and no refund may be possible for as eLearning materials that have already been issued.

GUIDED SHORE SNORKELLING

Indemnity, Liability Release and Assumption of Risk Agreement

I, the undersigned, acknowledge that guided shore snorkelling is an ocean-based activity and involves inherent risks and hazards. These risks may include, but are not limited to, changing sea and weather conditions, cold water, currents, surge, waves, reduced visibility, uneven or slippery entry and exit points, rocks, kelp, marine life, physical exertion, and the general risks associated with being in or near the ocean.

I am fully aware that these risks may result in personal injury, loss, damage, serious injury, or death.

I confirm that I am in good health and that I have truthfully disclosed any relevant medical conditions, concerns, allergies, medication use, swimming limitations, water-confidence concerns, or recent changes in my health that may affect my participation in the guided shore snorkelling activity.

I confirm that I am not currently suffering from any illness or condition that may affect my ability to safely participate in the activity, including but not limited to a cold, congestion, ear infection, breathing difficulty, dizziness, fainting, seizure-related condition, heart-related condition, or any condition that may affect my physical or mental ability in the water.

I understand that guided shore snorkelling may be physically strenuous and that I may be required to swim, float, walk over uneven ground, enter and exit the water from the shore, and participate in cold-water conditions.

I understand that the snorkelling site may be some distance from immediate professional medical care, and I still choose to participate in the activity with full knowledge of this.

I understand that all guided shore snorkelling activities are weather and condition dependent. The snorkelling site, meeting time, activity date, route, or activity plan may be changed, postponed, or cancelled by the Two Oceans Aquarium Dive School or the snorkelling guide for safety, operational, or environmental reasons.

I understand that the final meeting location and final activity arrangements will be confirmed closer to the activity date, once the Dive School has checked availability, weather, sea conditions, and operational requirements.

I agree to follow all safety briefings, rules, instructions, and directions given by the Two Oceans Aquarium Dive School, snorkelling guide, dive professionals, staff, or representatives before, during, and after the activity.

I understand that guided shore snorkelling is a supervised recreational activity, but that the ocean environment cannot be fully controlled. I voluntarily choose to participate and accept all risks associated with the activity, whether foreseen or unforeseen.

I hereby indemnify and hold harmless the Two Oceans Aquarium Trust (“the Aquarium”), its trustees, employees, independent contractors, agents, representatives, snorkelling guides, dive professionals, staff, and any person acting on behalf of the Aquarium in connection with the guided shore snorkelling activity, against any claim or claims by myself or by any other person, persons, or body whatsoever, for any injury, damage, loss, or death which may be sustained by me or any other person arising from my participation in the guided shore snorkelling activity.

I understand and agree that I am giving up certain legal rights by signing this agreement, including the right to bring a claim against the released parties for injury, loss, damage, or death arising from my participation, including where such loss may arise from negligence, whether passive or active.

I confirm that I am of lawful age and legally competent to sign this agreement. If the participant is under the age of 18, I confirm that I am the parent or legal guardian of the participant and that I am authorised to complete and sign this agreement on their behalf.

I have read and understood this indemnity, liability release, and assumption of risk agreement, and I voluntarily and willingly sign it to participate, or to allow the minor participant named in this booking form to participate, in the guided shore snorkelling activity

• It is essential that all medicals are finalized before your scheduled course. Should you have any uncertainties regarding the medical form, please do not hesitate to reach out to us in a timely manner.

• No refund policy: If a participant decides to withdraw or cancel 48 hours prior to the
experience, there will be no refunds..

• Rescheduling Policy: If you are unable to attend the expiernce on the scheduled dates, you must provide us with 48 hours' notice to reschedule. Failure to give notice or to attend on the scheduled dates may result in forfeiture of your fees.

• Weather Permitted: All diving activities are weather dependent. If the weather conditions are unfavourable for diving, the course may be rescheduled

• If weather conditions or other unforeseen circumstances prevent the dive from taking place, we will reschedule your experience or provide a full refund.Due to the nature of our operations, which are influenced by factors beyond our control
(including but not limited to weather conditions, safety considerations, and unforeseen circumstances), we reserve the right to amend or reschedule booking dates when necessary. While every effort will be made to adhere to the original schedule, clients acknowledge and accept that changes may be required, and we will communicate and arrange suitable alternative dates as promptly as possible.

• Due to the nature of recreational scuba diving and aquarium diving experiences, we may need to collect personal information about you, including information relating to your health and medical fitness to participate safely.

• We use Jotform to collect the required booking and participant information before your course or experience. This helps us make sure we have the correct details before your activity and reduces the need for multiple back-and-forth emails.

• Your privacy is very important to us. Unless we have your consent, we will not disclose your personally identifiable information or medical information to third parties. This information will only be used for the purposes directly related to your booking, course, experience, safety, and administration.

Please note that once your booking form has been submitted, the Dive School will review your request and revert back with the final confirmed course dates, as well as an invoice or payment link. Your booking will only be confirmed once payment has been received. If payment is made on-site, it must be completed before the commencement of the course.
