• 1. Introduction

  • Welcome to the AFC Stadium Information Form

    This Stadium Information Form captures key information relating to the stadium for the purpose of assessment in accordance with the AFC Stadium Regulations.

    Upon successful submission, an e-mail containing a link to download the AFC Full Stadium Information Form PDF will be sent to the e-mail address that you will provide in the form.

    The Form includes a Save and Continue function. Users must save their progress per page and use a stable internet connection when completing the form.

    Users are not required to register for an account or subscribe to any service in order to complete and submit the form. Simply click the Skip Create an Account option at the bottom of the pop-up box when it appears.

    Photo uploads are limited to 512KB per image (e.g. WhatsApp photos, etc.) to ensure smooth and stable submissions.

    Upon selecting Submit, users are requested to remain on the submission page until the confirmation is displayed.

    Kindly ensure that all required information is duly completed prior to submission.
