Enrolling in the Diaper Service

This agreement outlines the terms and conditions for participation in the Crèche Day Schools Diaper Service. 

Service Tiers & Pricing
The monthly fee for the diaper service is based on your child’s enrollment status:

• Full-Time Student $109.00
• Part-Time Student $89.00

Product Standards & Availability

• Primary Brand: Crèche utilizes Pampers Swaddlers diapers and Pampers Sensitive wipes.
• Substitutions: In the event of supply chain disruptions or inventory shortages, Crèche reserves the right to substitute these products with a brand of similar, premium quality.
• Absences and Closures: No refunds, credits, or reimbursements will be issued for student absences, family vacations, or school closures (including holidays, inclement weather, or scheduled closures).
• Size Changes: If your child outgrows their current size, a Size Change Request Form must be submitted. Crèche will accommodate the request as quickly as possible, based on inventory.

Enrollment & Proration

• New Enrollments: Families may join the service at any time by submitting an Enrollment Request form.
• Prorated Billing: Enrollments occurring on or before the 15th of the month will be billed at the full standard monthly rate. Enrollments occurring after the 15th will be prorated based on the remaining days of the month.

Cancellation Policy

• Notice Period: A notice of cancellation must be submitted by the 15th of the current month.
• Effective Date: Cancellations submitted by the 15th will take effect on the 1st of the following month.
• Late Notice: Requests received after the 15th will result in one final month of service and billing before the subscription is terminated.

Authorization & Agreement
By signing below, I acknowledge and agree to the following:

• I authorize Crèche Day Schools to charge my account the monthly fee associated with my child’s enrollment status.
• I understand that I am responsible for payment until a formal cancellation notice is submitted in accordance with the policy above.
• I understand that fees are non-refundable regardless of attendance or school operational status.
• I have read, understood, and agree to abide by all policies regarding the diaper service.

By submitting this form, you acknowledge and agree that you are responsible for payment until a cancellation request has been submitted in accordance with the policy outlined above.
