The Gospel for Kids Training

To jumpstart your studies at training camp, you are expected to complete the Gospel for Kids training before you arrive at Phase 2 (training camp). You learn about Gospel for Kids training at Phase 1.

To show that you have completed this training, when you arrive at Phase 2 please turn in your Wordless Book/Wristband coaching form (this form is included in the Gospel for Kids student notes).

Please note, this is a required part of your training.*

*Exception: Leadership Track students and most students who have completed the Gospel for Kids training at least twice do not need to review this training. If you're not sure whether you're required to complete this praining, please contact your local CEF staff.

Part 2: Medical Info
To help ensure that we can provide proper medical care, if needed, during Go Training and summer ministries, please provide the following information. (You'll need a parent/guardian signature.)
Please list any serious illnesses you have or have had that would need to be known in an emergency.
Please list any prescription medications you are taking. (Please note that you will turn these into the camp nurse when you arrive at Phase 2).
Please list any allergies/intolerances you have to foods
Please list any other allergies you have (medications, environmental, etc.)
Please briefly describe any medical or physical conditions that may require accommodation during training or summer ministries (Go Clubs, etc.). If you prefer, you may also contact your local CEF staff to discuss instead.

Part 3: Recreational Activities Waiver
During Phase 2, Cooperstown Bible Camp (CBC) may offer recreational activities that require a signed waiver before you can participate in these activities.

CBC Activities Waiver and Release of Liability

For All Participants:

I have read the waiver and release form and by signing below agree it is my intention to agree to its terms and to exempt and relieve CBC from any and all liability for personal injury, property damage, or wrongful death caused by negligence or any other cause.

The undersigned parent or guardian has read the CBC Activities Waiver and Release of Liability, agreeing to its terms and hereby gives permission for the above named minor to participate in Adventure Program activities at Cooperstown Bible Camp (CBC). Part 4: Registration Fee Payment
The balance of your registration fee is due when you arrive at Phase 2.

The total fee is $300, including a $30 deposit. (So, if you've already paid the deposit, your balance is $270).

Your balance may be less if you're recieving a scholarship or other funds.
