By signing this form, I authorize Musa’s Final Touch Barbershop (“MFT”) to photograph and/or record video of me during or after my appointment for marketing, promotional, and business purposes.
I understand and agree to the following:
1- MFT may use my photos and/or videos on:
• The official MFT website
• Social media platforms
• Online advertisements
• Printed marketing materials
• Google Business Profiles and other online directories
2- I understand that my image may be publicly visible online and may be viewed, shared, or reposted by third parties outside of MFT’s control.
3- I confirm that I voluntarily give permission to MFT to use my image, likeness, hairstyle, beard style, voice, and appearance for promotional purposes without compensation.
4- I understand that MFT may edit, crop, resize, or enhance photos/videos while maintaining professional presentation standards.
5- I release and hold harmless Musa’s Final Touch, its owners, employees, photographers, videographers, marketing partners, and affiliates from any claims related to the use of these photos/videos.
6- I confirm that:
• I am at least 18 years old, OR
• I am the parent/legal guardian signing on behalf of a minor.
7- I understand that I may request removal of my photo/video from future marketing materials by contacting MFT directly. However, I understand that previously published or shared content may not be fully removable from the internet.
