Assumption of Risk:
I understand that participation in obstacle courses and physical activities involves inherent risks, including but not limited to falls, collisions, slips, equipment failure, and other potential injuries. I voluntarily allow my child to participate and assume all risks associated with these activities.
Release of Liability:
I, on behalf of myself and my child, hereby release, waive, and discharge Camp 1315, its owners, employees, contractors, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, or damage sustained while participating in activities at Camp 1315.
Behavior & Safety Agreement:
I understand that my child is expected to follow all rules and instructions provided by Camp 1315 staff. Failure to comply may result in removal from activities.
