Are you open to other FMN pet suggestions if not selected for your specified pet? (NOTE: this is not a guarantee that FMN will reach out for other pets, but let's us know if you'd potentially be interested. This is mostly used for litters or when we have multiple similar pets)

Are you able and willing to enroll the pet in training? Training is necessary for all puppies in order to thrive and grow into a well behaved dog. Training may be necessary for older dogs based on what their previous owners have already completed.

Pets are a lifelong commitment. Dog can live 10-12 years (sometimes more!) and cats even longer. Are you prepared to provide care to your new pet for the remainder of their life? To provide it with quality food, vet care, and unconditional love?

I understand FMN is a volunteer-ran organization and will only reach out if I am selected to meet with the pet I have selected on this application, I will need to reach out to have this application applied to other pets I am interested in the future

I confirm I am financially, physically, and emotionally ready to adopt a pet! I understand that if approved & selected and I chose to adopt the pet during the scheduled meet & greet, I will be expected to be fully prepared to take home the applied for pet at that time. FMN is unable to provide payments, “hold” pets, or provide delayed pickup. FMN does not offer "foster to adopt" or trial period. This is for the benefit of our pets, to keep them out of unnecessary limbo, to maintain an efficient and effective adoption process, and to be considerate of our volunteers’ time, effort, and energy.
