PEORIA FASHION WEEK ACADEMY
Media / Photo / Video Release Waiver
For Designers, Models, Participants, and Creative Contributors

This Media/Photo/Video Release Waiver is entered into by the undersigned participant and Peoria Fashion Week Academy, including its representatives, staff, volunteers, contractors, photographers, videographers, media partners, sponsors, and affiliates.

By signing this waiver, I understand and agree to the following:

1. Permission to Capture Media
I grant Peoria Fashion Week Academy permission to photograph, record, film, livestream, interview, and otherwise capture my image, likeness, voice, name, performance, designs, garments, creative work, runway appearance, backstage participation, interviews, and event-related activities during Peoria Fashion Week Academy programs, fashion shows, classes, castings, rehearsals, workshops, photo shoots, promotional events, and related activities.

2. Permission to Use Media
I authorize Peoria Fashion Week Academy to use, publish, display, reproduce, edit, distribute, and share photos, videos, audio recordings, livestreams, interviews, and other media that include me or my creative work for promotional, educational, archival, fundraising, marketing, advertising, press, social media, website, grant reporting, sponsor reporting, and community outreach purposes.

This may include, but is not limited to:

Facebook, Instagram, TikTok, YouTube, websites, printed flyers, magazines, press releases, news coverage, sponsorship packets, grant reports, presentations, promotional videos, event recaps, and future marketing materials.

3. No Compensation
I understand that I will not receive payment, royalties, or additional compensation for the use of my image, likeness, voice, designs, garments, performance, or creative work in any media captured or used by Peoria Fashion Week Academy.

4. Ownership and Usage Rights
I understand that all photos, videos, recordings, and media captured by Peoria Fashion Week Academy or its approved media team are the property of Peoria Fashion Week Academy and/or the approved photographer, videographer, or media creator.

I understand that Peoria Fashion Week Academy may continue to use this media now and in the future without needing additional approval from me.

5. Designers’ Creative Work
As a designer, I understand that my garments, designs, styling, and creative work may be photographed, filmed, posted, published, and promoted as part of Peoria Fashion Week Academy events and programming.

Peoria Fashion Week Academy agrees to make reasonable efforts to credit designers when appropriate and possible, especially in official promotional materials, social media posts, event recaps, and publications.

6. Models’ Participation
As a model, I understand that my runway walk, posing, backstage participation, fittings, rehearsals, interviews, and photo/video appearances may be captured and used for promotional and archival purposes.

I understand that participation in Peoria Fashion Week Academy events may result in my image appearing in public-facing media.

7. Release of Claims
I release Peoria Fashion Week Academy, its staff, board members, volunteers, contractors, media team, photographers, videographers, sponsors, partners, and affiliates from any claims, demands, damages, or liability related to the use of my image, likeness, voice, performance, creative work, or participation in media.

This includes any claims related to privacy, publicity rights, copyright concerns regarding captured event media, or the use of my image in promotional materials.

8. Accuracy and Editing
I understand that photos, videos, audio, and interviews may be edited, cropped, captioned, combined with other content, or used in promotional materials. Peoria Fashion Week Academy will make reasonable efforts to represent participants respectfully and professionally.

9. Minor Participants
For participants under the age of 18, a parent or legal guardian must sign this waiver. By signing, the parent or guardian gives permission for the minor participant to be photographed, filmed, recorded, and included in Peoria Fashion Week Academy media and promotional materials.

10. Agreement
By signing below, I confirm that I have read and understand this Media/Photo/Video Release Waiver. I voluntarily agree to its terms.

PEORIA FASHION WEEK ACADEMY
General Liability Waiver and Release Form
This General Liability Waiver and Release Form is entered into by the undersigned participant and Peoria Fashion Week Academy, including its board members, officers, staff, instructors, volunteers, contractors, partners, sponsors, affiliates, representatives, and event organizers.

By signing this waiver, I acknowledge, understand, and agree to the following:

1. Participation Acknowledgment
I voluntarily agree to participate in Peoria Fashion Week Academy programs, events, classes, workshops, rehearsals, fittings, fashion shows, castings, photo shoots, travel activities, vendor events, community events, and related activities.

I understand that participation may involve physical movement, walking, modeling, dancing, sewing, garment construction, use of tools or equipment, standing for long periods, carrying items, travel, interaction with other participants, and participation in public or private event spaces.

2. Assumption of Risk
I understand that participation in Peoria Fashion Week Academy activities may involve certain risks, including but not limited to:

Slips, trips, falls, or physical injury
Injury from sewing tools, scissors, needles, pins, irons, steamers, machines, or equipment
Wardrobe, garment, or footwear-related accidents
Stage, runway, backstage, rehearsal, or fitting-room accidents
Allergic reactions, illness, or exposure to communicable diseases
Emotional discomfort, stress, or performance-related pressure
Property loss, theft, or damage
Travel-related incidents, where applicable
Risks associated with public events, large gatherings, or third-party venues
I knowingly and voluntarily assume all risks connected to my participation.

3. Release of Liability
I release and hold harmless Peoria Fashion Week Academy, its board members, officers, staff, instructors, volunteers, contractors, sponsors, partners, affiliates, representatives, event organizers, and venue partners from any claims, demands, damages, losses, injuries, expenses, or liability arising out of or related to my participation.

This includes claims related to personal injury, illness, property damage, lost or stolen property, emotional distress, accidents, negligence, or participation in any Peoria Fashion Week Academy activity or event.

4. Medical Treatment Authorization
In the event of an emergency, I authorize Peoria Fashion Week Academy staff, volunteers, or representatives to seek emergency medical assistance on my behalf or on behalf of my minor child, if applicable.

I understand that Peoria Fashion Week Academy is not responsible for medical costs, ambulance fees, hospital bills, treatment expenses, or related healthcare charges.

5. Personal Property
I understand that I am responsible for my own belongings, including clothing, garments, sewing supplies, equipment, phones, bags, jewelry, electronics, money, and personal items.

Peoria Fashion Week Academy is not responsible for lost, stolen, misplaced, or damaged personal property.

6. Code of Conduct
I agree to conduct myself in a respectful, professional, and safe manner while participating in Peoria Fashion Week Academy activities.

I understand that unsafe behavior, harassment, bullying, discrimination, violence, threats, theft, substance use, disruptive conduct, or failure to follow staff instructions may result in removal from the program or event without refund, when applicable.

7. Supervision of Minors
For participants under the age of 18, a parent or legal guardian must sign this waiver.

Parents and guardians understand that Peoria Fashion Week Academy will provide reasonable supervision during official program hours, but parents/guardians remain responsible for transportation, timely pickup, emergency contact information, medical disclosures, and any special needs or safety concerns involving their child.

8. Transportation and Travel
If transportation or travel is involved, I understand that travel may include additional risks. I release Peoria Fashion Week Academy from liability related to transportation incidents unless otherwise required by law.

If I provide my own transportation or allow my child to be transported by someone else, I understand that Peoria Fashion Week Academy is not responsible for incidents that occur outside official Academy-supervised activities.

9. Third-Party Venues and Partners
I understand that Peoria Fashion Week Academy may hold activities at third-party venues, including but not limited to schools, hotels, event centers, community spaces, parks, studios, or partner locations.

I agree to follow all venue rules, safety procedures, and instructions from Peoria Fashion Week Academy staff and venue representatives.

10. Agreement
By signing below, I confirm that I have read and understand this General Liability Waiver and Release Form. I understand that I am giving up certain legal rights by signing this waiver. I sign this agreement voluntarily.
