I, the undersigned, on behalf of myself and/or my minor child, acknowledge that participation in the Youth Football Summer Camp hosted by Pretty Fit Wellness LLC and J SWAG Agency involves inherent risks, including but not limited to physical injury, illness, or accidents that may arise from activities such as speed training, agility drills, conditioning, mobility, flexibility, and other athletic exercises. I voluntarily assume all risks associated with participation and agree to release, waive, and hold harmless Pretty Fit Wellness LLC, J SWAG Agency, all coaches, trainers, staff, volunteers, affiliates, and First Baptist Church of Progress Village, including its owners, employees, and representatives, from any and all liability, claims, demands, or causes of action arising out of or related to any injury, loss, or damage incurred during participation. I certify that I/my child is physically able to participate and authorize staff to seek emergency medical treatment if necessary, understanding that I am responsible for any related expenses. I also grant permission for Pretty Fit Wellness LLC and J SWAG Agency to capture and use photos, videos, and recordings of myself/my child for marketing, promotional, and social media purposes without compensation. I understand that all payments are non-refundable and that participants are expected to follow all rules and instructions, with the understanding that failure to do so may result in dismissal without refund. By signing below, I confirm that I have read, understood, and agree to all terms outlined in this waiver.
