I, the undersigned, voluntarily agree to participate (or allow my child to participate) in fitness and/or boxing training sessions, classes, or related activities (“Activities”) provided by TRU Sports Training Club and its trainers, coaches, staff, affiliates, and representatives (“Trainer”).

I understand and acknowledge the following:

1. Assumption of Risk:
I understand that participation in fitness and boxing activities involves inherent risks, including but not limited to physical exertion, contact with equipment or other individuals, risk of injury, illness, muscle strains, fractures, heart attacks, or other serious or minor injuries. I voluntarily assume full responsibility for all risks, known and unknown, associated with my (or my child’s) participation in any and all activities with TRU Sports.

2. Release of Liability:
In consideration of being allowed to participate in any way with TRU Sports, I hereby release, waive, and forever discharge TRU Sports, its owners, employees, contractors, trainers, and affiliates from any and all liability, claims, demands, or causes of action arising from or related to any injury, illness, or damages sustained by me (or my child) while participating in activities, using equipment, or being present at training locations—whether caused by the negligence of TRU Sports or otherwise.

3. Medical Authorization:
I certify that I (or my child) am physically fit and capable of safely participating in fitness and boxing training. I acknowledge that it is my responsibility to consult with a physician prior to beginning any exercise or training program. I authorize TRU Sports to seek emergency medical treatment on my behalf (or on behalf of my child) in the event of an accident or injury, and I accept full financial responsibility for any associated costs.

4. Indemnification:
I agree to indemnify and hold harmless TRU Sports from and against any and all liabilities, damages, losses, claims, expenses (including attorneys’ fees), or actions that arise as a result of my (or my child’s) participation in the Activities.

5. Minor Participants:
If the participant is under the age of 18, I certify that I am the legal parent or guardian and have the legal authority to sign this waiver on their behalf. I understand and agree to all terms as they apply to my child’s participation.

6. Acknowledgment of Understanding:
I have read this Waiver and Release of Liability carefully and fully understand its contents. I understand that by signing this form, I am giving up legal rights to sue and am accepting all responsibility for any risks or outcomes associated with the services provided by TRU Sports.
