      IMPORTANT NOTICE

      This document affects your legal rights. Please read carefully before signing.

      SCOPE OF AGREEMENT

      This agreement applies to all activities associated with Coach Clarke's Archery LLC, including but not limited to lessons, classes, camps, clinics, tournaments, leagues, competitions, practices, demonstrations, training sessions, travel activities, and other events whether conducted, organized, sponsored, attended, or participated in through Coach Clarke's Archery LLC.

      ASSUMPTION OF RISK

      I understand and acknowledge that participation in archery activities involves inherent and significant risks including, but not limited to:

      • Arrow-related injuries

      • Equipment malfunction or failure

      • Accidental release or discharge of bows or arrows

      • Injuries caused by participants, spectators, instructors, coaches, volunteers, or third parties

      • Slips, trips, falls, collisions, or other accidents

      • Eye injuries

      • Muscle strains or physical exertion injuries

      • Property damage

      • Permanent disability

      • Paralysis

      • Serious bodily injury

      • Death

      I understand that these risks may arise from my own actions, the actions of others, equipment failure, facility conditions, travel, negligence, or other known or unknown causes.

      I voluntarily choose to participate in activities associated with Coach Clarke's Archery LLC and knowingly and freely assume all risks, both known and unknown, even if arising from negligence, to the fullest extent permitted by law.

      RELEASE OF LIABILITY AND WAIVER

      In consideration for being allowed to participate in activities associated with Coach Clarke's Archery LLC, I hereby release, waive, discharge, and covenant not to sue Coach Clarke's Archery LLC, Joshua Clarke, instructors, coaches, volunteers, employees, representatives, landlords, property owners, sponsors, affiliates, agents, host facilities, event organizers, and any associated parties (collectively referred to as the "Released Parties") from any and all liability, claims, demands, damages, losses, causes of action, or expenses arising out of or related to any injury, disability, death, property damage, or other loss that may occur as a result of participation in activities.

      This release applies whether caused by negligence of the Released Parties or otherwise, to the fullest extent permitted by Alabama law.

      I understand and agree that this waiver is intended to be as broad and inclusive as permitted by the laws of the State of Alabama.

      UNKNOWN OR UNFORESEEN INJURIES

      I understand that injuries, damages, or losses may occur that are not presently known or anticipated.

      I expressly waive any claims arising from such unknown or unforeseen injuries, damages, or losses to the fullest extent permitted by law.

      PARTICIPANT CONDUCT

      Participants are expected to behave in a safe, respectful, and sportsmanlike manner at all times.

      Unsafe behavior, harassment, bullying, intimidation, disruptive conduct, refusal to follow instructions, misuse of equipment, or conduct that interferes with the safety or enjoyment of others may result in immediate removal from activities without refund.

      Coach Clarke's Archery LLC reserves the right to suspend or terminate participation when deemed necessary for safety, instruction, or the well-being of participants.

      MEDICAL CONDITIONS AND SPECIAL CONSIDERATIONS

      Participants agree to disclose any medical conditions, allergies, injuries, physical limitations, behavioral considerations, developmental conditions, learning differences, or other factors that may affect participation or instruction.

      This information assists Coach Clarke's Archery LLC in providing a safer environment and tailoring instruction to meet the individual needs of each participant.

      Failure to disclose relevant information may limit Coach Clarke's Archery LLC's ability to provide appropriate instruction, accommodations, or emergency assistance.

      PERSONAL EQUIPMENT

      Participants who choose to use their own bows, arrows, releases, sights, stabilizers, quivers, or other equipment do so at their own risk.

      Coach Clarke's Archery LLC is not responsible for damage, loss, theft, malfunction, or breakage of personal equipment occurring before, during, or after participation in any activity.

      TRANSPORTATION

      Unless otherwise agreed in writing, participants are responsible for their own transportation to and from lessons, practices, camps, tournaments, competitions, and other activities.

      Coach Clarke's Archery LLC assumes no responsibility for transportation arrangements made by participants or third parties.

      TRAVEL AND OFF-SITE ACTIVITIES

      I understand that participation may include attendance at off-site practices, tournaments, competitions, camps, clinics, demonstrations, or other activities associated with Coach Clarke's Archery LLC.

      I understand that such activities may occur at facilities, venues, ranges, parks, schools, or locations not owned, operated, or controlled by Coach Clarke's Archery LLC.

      I acknowledge that participation in off-site activities may involve additional risks, including travel-related risks, unfamiliar facilities, weather conditions, and circumstances beyond the control of Coach Clarke's Archery LLC.

      I voluntarily assume all such risks associated with participation in off-site activities.

      MEDICAL AUTHORIZATION

      I certify that I am physically able to participate in archery activities.

      I authorize Coach Clarke's Archery LLC and its representatives to obtain emergency medical treatment if deemed necessary.

      I understand and agree that I am solely financially responsible for any medical treatment, transportation, or related expenses incurred.

      PHOTO AND VIDEO RELEASE

      As a condition of participation, I grant Coach Clarke's Archery LLC permission to photograph, video record, and otherwise capture my likeness during activities.

      I authorize Coach Clarke's Archery LLC to use photographs, video recordings, images, likeness, voice, and name for promotional, advertising, marketing, educational, website, social media, and business purposes without compensation or additional approval.

      NO GUARANTEE OF SAFETY

      I understand that while reasonable safety measures, supervision, and instruction are provided, Coach Clarke's Archery LLC cannot guarantee the safety of participants at all times.

      I acknowledge that accidents may occur despite safety procedures and supervision.

      ELECTRONIC SIGNATURES

      By electronically signing and submitting this form, I acknowledge that my electronic signature is legally binding and has the same force and effect as an original handwritten signature.

      SEVERABILITY

      If any portion of this agreement is found to be invalid or unenforceable, the remaining portions shall remain in full force and effect.

      GOVERNING LAW AND VENUE

      This agreement shall be governed by and interpreted under the laws of the State of Alabama.

      Any legal action arising from participation in activities associated with Coach Clarke's Archery LLC shall be brought exclusively in Mobile County, Alabama.

      ENTIRE AGREEMENT

      This agreement constitutes the entire agreement between the parties and supersedes all prior discussions, understandings, representations, or agreements relating to the subject matter herein.

      No modification or waiver of any provision shall be effective unless made in writing by Coach Clarke's Archery LLC.

      ACKNOWLEDGMENT OF UNDERSTANDING

      By signing below, I acknowledge that:

      • I have read this entire agreement.

      • I understand its terms.

      • I understand that I am giving up substantial legal rights.

      • I understand the risks associated with participation.

      • I sign this agreement freely and voluntarily.

      • No oral representations or statements have been made to alter the terms of this agreement.

    • MINOR WAIVER & PARENT/GUARDIAN CONSENT AGREEMENT

      IMPORTANT NOTICE

      This document affects your legal rights and the legal rights of the minor participant. Please read carefully before signing.

      SCOPE OF AGREEMENT

      This agreement applies to all activities associated with Coach Clarke's Archery LLC, including but not limited to lessons, classes, camps, clinics, tournaments, leagues, competitions, practices, demonstrations, training sessions, travel activities, and other events whether conducted, organized, sponsored, attended, or participated in through Coach Clarke's Archery LLC.

      ASSUMPTION OF RISK

      I understand and acknowledge that participation in archery activities involves inherent and significant risks including, but not limited to:

      • Arrow-related injuries

      • Equipment malfunction or failure

      • Injuries caused by participants, spectators, instructors, coaches, volunteers, or third parties

      I voluntarily allow the minor participant to participate in activities offered through Coach Clarke's Archery LLC and knowingly and freely assume all risks, both known and unknown, even if arising from negligence, to the fullest extent permitted by law.

      RELEASE OF LIABILITY AND WAIVER

      In consideration for allowing the minor participant to participate in activities associated with Coach Clarke's Archery LLC, I hereby release, waive, discharge, and covenant not to sue Coach Clarke's Archery LLC, Joshua Clarke, instructors, coaches, volunteers, employees, representatives, landlords, property owners, sponsors, affiliates, agents, host facilities, event organizers, and any associated parties (collectively referred to as the "Released Parties") from any and all liability, claims, demands, damages, losses, causes of action, or expenses arising out of or related to any injury, disability, death, property damage, or other loss that may occur as a result of participation in activities.

      This release applies whether caused by negligence of the Released Parties or otherwise, to the fullest extent permitted by Alabama law.

      I understand and agree that this waiver is intended to be as broad and inclusive as permitted by the laws of the State of Alabama.

      UNKNOWN OR UNFORESEEN INJURIES

      PARENT/GUARDIAN CONDUCT

      Parents and guardians are expected to maintain respectful and appropriate behavior while attending lessons, camps, tournaments, competitions, or other activities.

      Parents and guardians may observe activities when appropriate but must not interfere with instruction, participant focus, range operations, or safety procedures.

      Harassment, abusive language, threats, interference with instruction, disruptive behavior, or failure to follow facility rules may result in removal from the facility and/or suspension or termination of participation without refund.

      MEDICAL CONDITIONS AND SPECIAL CONSIDERATIONS

      Parents and guardians agree to disclose any medical conditions, allergies, injuries, physical limitations, behavioral considerations, developmental conditions, learning differences, or other factors that may affect participation or instruction.

      This information assists Coach Clarke's Archery LLC in providing a safer environment and tailoring instruction to meet the individual needs of each participant.

      PERSONAL EQUIPMENT

      Coach Clarke's Archery LLC is not responsible for damage, loss, theft, malfunction, or breakage of personal equipment occurring before, during, or after participation in any activity.

      SUPERVISION AND PICKUP AUTHORIZATION

      I understand that Coach Clarke's Archery LLC provides archery instruction, coaching, and related activities and is not a childcare provider or daycare facility.

      I understand that parents and guardians are responsible for ensuring that participants arrive on time and are picked up promptly at the conclusion of scheduled activities.

      I authorize my child to participate in activities without my physical presence during the scheduled activity period.

      Coach Clarke's Archery LLC shall not be responsible for supervising participants before scheduled activities begin or after scheduled activities have concluded.

      Parents and guardians are responsible for informing Coach Clarke's Archery LLC of any authorized pickup arrangements, emergency contacts, or special circumstances regarding participant supervision.

      Repeated late pickups may result in suspension or termination of participation without refund.

      TRANSPORTATION

      Unless otherwise agreed in writing, participants and parents/guardians are responsible for their own transportation to and from lessons, practices, camps, tournaments, competitions, and other activities.

      Coach Clarke's Archery LLC assumes no responsibility for transportation arrangements made by participants, parents, guardians, or third parties.

      TRAVEL AND OFF-SITE ACTIVITIES

      I understand and agree that I am solely financially responsible for any medical treatment, transportation, or related expenses incurred.

      PHOTO AND VIDEO RELEASE

      As a condition of participation, I grant Coach Clarke's Archery LLC permission to photograph, video record, and otherwise capture the participant's likeness during activities.

      I authorize Coach Clarke's Archery LLC to use photographs, video recordings, images, likeness, voice, and name for promotional, advertising, marketing, educational, website, social media, and business purposes without compensation or additional approval.

      I voluntarily sign this agreement and consent to the minor participant's participation in activities associated with Coach Clarke's Archery LLC.

      ELECTRONIC SIGNATURES

      By electronically signing and submitting this form, the parent or guardian acknowledges that their electronic signature is legally binding and has the same force and effect as an original handwritten signature.

      SEVERABILITY

      If any portion of this agreement is found to be invalid or unenforceable, the remaining portions shall remain in full force and effect.

      GOVERNING LAW AND VENUE

      This agreement shall be governed by and interpreted under the laws of the State of Alabama.

      Any legal action arising from participation in activities associated with Coach Clarke's Archery LLC shall be brought exclusively in Mobile County, Alabama.

    • Monthly Lesson Agreement 
    • MONTHLY ENROLLMENT

      Enrollment in Coach Clarke’s Archery LLC monthly lesson program reserves recurring instructional time and coaching availability for the participant.

      Monthly enrollment includes scheduled lessons as agreed upon between Coach Clarke’s Archery LLC and the participant and/or parent/guardian.

      INITIAL COMMITMENT

      Monthly lesson enrollment may require an initial three (3) month commitment depending on the selected enrollment plan.

      By enrolling in a commitment-based plan, the participant and/or parent/guardian agrees to pay all tuition due during the initial commitment period.

      Early cancellation during the initial commitment period does not release the participant and/or parent/guardian from responsibility for any remaining tuition balance owed under this agreement.

      In the event collection efforts, legal action, or enforcement of this agreement becomes necessary, the participant and/or parent/guardian agrees to be responsible for all reasonable collection costs, court costs, attorney fees, and related expenses incurred by Coach Clarke’s Archery LLC to the fullest extent permitted by law.

      MONTH-TO-MONTH ENROLLMENT

      Following completion of any initial commitment period, enrollment shall continue on a month-to-month basis unless otherwise agreed in writing.

      Participants may cancel enrollment by providing at least seven (7) days written notice prior to the next scheduled billing date.

      Notice may be provided by text message, email, or other written communication accepted by Coach Clarke’s Archery LLC.

      Tuition already paid is non-refundable.

      TUITION AND PAYMENT

      Monthly tuition is due on the 1st day of each month.

      Full payment must be received on or before the due date to maintain active enrollment status.

      Monthly tuition reserves recurring lesson times and coaching availability for the participant.

      Tuition is based upon reserving instructional time and maintaining enrollment, not solely upon attendance.

      Missed lessons, absences, vacations, scheduling conflicts, suspensions resulting from non-payment, or other participant-related circumstances do not reduce tuition obligations except as expressly provided by Coach Clarke’s Archery LLC.

      LATE PAYMENTS AND SUSPENSION OF SERVICES

      A $25 late fee will be assessed on any balance not paid by the 10th day of the month.

      If payment has not been received by the 15th day of the month, Coach Clarke’s Archery LLC reserves the right to suspend lessons, coaching services, camps, tournaments, events, programs, and all participation privileges until the account is brought current.

      Suspension of services due to non-payment does not cancel, reduce, or waive any tuition, fees, or financial obligations owed under this agreement.

      The participant and/or parent/guardian remains fully responsible for all tuition, late fees, outstanding balances, and other amounts due, including the month during which services are suspended.

      Any returned check, declined payment, chargeback, failed electronic payment, or other reversed transaction may be subject to a returned payment fee equal to the actual fees incurred by Coach Clarke’s Archery LLC or the maximum amount permitted by law.

      LESSON ATTENDANCE

      Participants are expected to provide at least twenty-four (24) hours notice if they are unable to attend a scheduled lesson.

      Participants who fail to attend a scheduled lesson without notice shall be considered a no-show.

      No-show lessons are forfeited and are not eligible for makeup lessons, credits, refunds, or rescheduling.

      Coach Clarke’s Archery LLC reserves the right to determine eligibility for makeup lessons in cases involving emergencies or extraordinary circumstances.

      MAKEUP LESSONS

      Participants who provide at least twenty-four (24) hours notice of an absence may be eligible for a makeup lesson based on scheduling availability.

      Participants may accumulate a maximum of four (4) makeup lessons at any given time.

      Makeup lessons should be completed within thirty (30) days of the missed lesson whenever reasonably possible.

      Makeup lessons not completed within a reasonable period of time, as determined by Coach Clarke’s Archery LLC, may expire.

      Once four (4) makeup lessons have accumulated, any additional missed lessons are forfeited unless the absence results from a cancellation initiated by Coach Clarke’s Archery LLC.

      Makeup lessons have no cash value, are non-transferable, and may not be exchanged for refunds, credits, or cash.

      Coach Clarke’s Archery LLC reserves the right to determine scheduling availability for all makeup lessons.

      TEMPORARY ABSENCES

      Participants may temporarily miss lessons due to vacations, travel, school activities, illness, or other scheduling conflicts.

      Makeup lessons must be completed within the same calendar month whenever reasonably possible.

      Unused lessons do not roll over indefinitely and have no cash value.

      PARENT/GUARDIAN OBSERVATION AND CONDUCT

      Parents and guardians are welcome to observe lessons when appropriate; however, they must remain in designated areas and avoid interfering with instruction, participant focus, range operations, or safety procedures.

      Parents and guardians shall not coach, direct, distract, or otherwise interfere with participants during instruction unless specifically requested by Coach Clarke’s Archery LLC.

      For the safety of all participants, parents and guardians must remain clear of shooting areas, equipment areas, and instructional spaces unless authorized by Coach Clarke’s Archery LLC.

      Parents and guardians are not required to remain present for the duration of lessons unless requested by Coach Clarke’s Archery LLC.

      Disruptive behavior, interference with instruction, unsafe conduct, harassment, or failure to follow facility rules may result in removal from the facility and/or suspension or termination of participation without refund.

      COMMUNICATION

      Participants and/or parents/guardians are responsible for maintaining current contact information.

      Important notices, scheduling updates, billing reminders, cancellations, and program communications may be provided by phone, text message, email, or other electronic means.

      Delivery of such communication shall be deemed sufficient notice.

      ELECTRONIC SIGNATURES

      By electronically signing and submitting this form, the participant and/or parent/guardian acknowledges that their electronic signature is legally binding and has the same force and effect as an original handwritten signature.

      CHARGEBACKS AND PAYMENT DISPUTES

      Participants and/or parents/guardians agree to contact Coach Clarke’s Archery LLC regarding any billing concerns before initiating a chargeback, payment dispute, or payment reversal through a financial institution or payment processor.

      Initiating a chargeback or payment dispute does not relieve the participant and/or parent/guardian of obligations under this agreement.

      ENTIRE AGREEMENT

      This agreement constitutes the entire agreement between the parties and supersedes all prior discussions, understandings, representations, or agreements relating to the subject matter herein.

      No modification or waiver of any provision shall be effective unless made in writing by Coach Clarke’s Archery LLC.

      ACKNOWLEDGMENT

      By signing below, I acknowledge that:

      • I have read this entire agreement.

      • I understand and agree to all terms and conditions contained herein.

      • I understand my financial obligations under this agreement.

      • I understand that missed lessons, late payments, and cancellations are subject to the policies outlined above.

      • I voluntarily agree to be bound by this agreement.

    • Individual Lesson Contract 
    • INDIVIDUAL LESSON REGISTRATION AND PAYMENT

      By registering for individual lessons at Coach Clarke’s Archery LLC, the participant and/or parent/guardian agrees to the following terms and conditions.

      Lesson rates vary depending on lesson type, duration, and availability. Current pricing and scheduling information is available directly through Coach Clarke’s Archery LLC.

      Full payment is required at the time of booking in order to reserve and confirm a lesson time.

      A $25 booking deposit is included in the lesson fee and reserves the participant's scheduled lesson time.

      CANCELLATIONS AND RESCHEDULING

      Participants may reschedule a lesson by providing at least twenty-four (24) hours notice prior to the scheduled lesson time.

      Lessons canceled with less than twenty-four (24) hours notice will result in forfeiture of the $25 booking deposit.

      Any remaining lesson credit may be applied toward a rescheduled lesson.

      Rescheduled lessons must be completed within fourteen (14) days of the original lesson date.

      If the lesson is not rescheduled and completed within fourteen (14) days, any remaining lesson credit shall expire and be forfeited.

      NO-SHOW POLICY

      Participants who fail to attend a scheduled lesson without notice shall be considered a no-show.

      No-show lessons are forfeited in their entirety.

      No refunds, credits, makeup lessons, or rescheduling opportunities will be provided for no-show appointments.

      LATE ARRIVALS

      Participants arriving late will receive the remainder of their scheduled lesson time.

      Lessons will end at the originally scheduled time regardless of participant arrival time.

      No refunds, credits, or extensions will be provided for late arrivals.

      EMERGENCIES

      Coach Clarke’s Archery LLC understands that genuine emergencies may occur.

      Exceptions to cancellation, rescheduling, or forfeiture policies may be granted at the sole discretion of Coach Clarke’s Archery LLC.

      COACH RESCHEDULING OR CANCELLATION

      Coach Clarke’s Archery LLC reserves the right to reschedule or cancel lessons due to emergencies, illness, facility conflicts, weather conditions, tournaments, staffing conflicts, or other unforeseen circumstances.

      If Coach Clarke’s Archery LLC cancels a lesson, the participant may choose either:

      • A full refund of all amounts paid, including the booking deposit; or

      • Rescheduling of the lesson to a mutually agreeable date and time.

      Reasonable efforts will be made to provide advance notice whenever possible.

      PARENT/GUARDIAN OBSERVATION AND CONDUCT

      Parents and guardians are welcome to observe individual lessons when appropriate.

      Parents and guardians must remain respectful of the instructional process and may not interfere with coaching, participant focus, range operations, or safety procedures.

      Parents and guardians shall not coach, direct, distract, or otherwise interfere with participants during instruction unless specifically requested by Coach Clarke’s Archery LLC.

      For safety reasons, parents and guardians must follow all directions provided by Coach Clarke’s Archery LLC while on the range or within the facility.

      Disruptive behavior, interference with instruction, unsafe conduct, harassment, or failure to follow facility rules may result in removal from the facility and/or termination of the lesson without refund.

      COMMUNICATION

      Participants and/or parents/guardians are responsible for maintaining current contact information.

      Important notices, scheduling updates, cancellations, and lesson communications may be provided by phone, text message, email, or other electronic means.

      Delivery of such communication shall be deemed sufficient notice.

      CHARGEBACKS AND PAYMENT DISPUTES

      Participants and/or parents/guardians agree to contact Coach Clarke’s Archery LLC regarding any billing concerns before initiating a chargeback, payment dispute, or payment reversal through a financial institution or payment processor.

      Initiating a chargeback or payment dispute does not relieve the participant and/or parent/guardian of obligations under this agreement.

      ELECTRONIC SIGNATURES

      By electronically signing and submitting this form, the participant and/or parent/guardian acknowledges that their electronic signature is legally binding and has the same force and effect as an original handwritten signature.

      ENTIRE AGREEMENT

      This agreement constitutes the entire agreement between the parties and supersedes all prior discussions, understandings, representations, or agreements relating to the subject matter herein.

      No modification or waiver of any provision shall be effective unless made in writing by Coach Clarke’s Archery LLC.

      ACKNOWLEDGMENT

      By signing below, I acknowledge that:

      • I have read this entire agreement.

      • I understand and agree to all terms and conditions contained herein.

      • I understand the cancellation, rescheduling, no-show, and payment policies.

      • I understand that lesson payments are subject to the terms outlined above.

      • I voluntarily agree to be bound by this agreement.

      Coach Clarke’s Archery LLC (“CCA”) provides online coaching and athlete-development services, currently including CCA Progress Studio and Athlete Mental Reflection.

      This section is separate from the general participation waiver and applies to the athlete’s use of CCA’s online coaching and athlete-development features.

      Athlete Mental Reflection

      Athlete Mental Reflection is a coaching-development tool designed to help athletes think through practices and tournaments, including their preparation, focus, shot process, distractions, recovery and reset decisions, and what went well.

      Athlete Mental Reflection is not medical care, therapy, psychological treatment, diagnosis, or an emergency service.

      Athletes may provide written responses in their own words.

      Privacy of Reflection

      Detailed Athlete Mental Reflection responses are intended to remain private between the athlete and authorized CCA coaching access by default.

      Parent or guardian access to other CCA information does not automatically include access to:

      * the athlete’s raw Mental Reflection responses;
      * detailed Coach responses or follow-up questions;
      * private Coach notes; or
      * automatic notice that a particular private Reflection exists or has been completed.

      CCA may separately communicate appropriate athlete-development information with a parent or guardian when needed.

      CCA cannot promise absolute confidentiality. Information may need to be shared when reasonably necessary or legally required for athlete safety, welfare, protection, or another lawful purpose.

      Athlete Online Account

      The athlete may receive access to CCA’s online coaching and athlete-development services through a CCA account, including a CCA username and password.

      The athlete’s email address is not required to be the login identity. Parent or guardian contact information may be used for approved communication, account recovery, consent, or other authorized purposes.

      Helping with account recovery does not automatically give a parent or guardian access to athlete-private Mental Reflection content.

      Information That May Be Collected

      Depending on the CCA services and features used, information may include:

      * athlete name and CCA Student identity;
      * age or date of birth;
      * account and access information;
      * practice and tournament information;
      * coaching-development information;
      * Athlete Mental Reflection responses;
      * Coach responses and follow-up questions;
      * practice videos when separately submitted through CCA’s Practice workflow; and
      * security, consent, and audit information needed to operate the service.

      CCA does not intend to sell athlete information or use private athlete information for targeted advertising.

      Safety

      Athlete Mental Reflection is not continuously monitored.

      An athlete who needs immediate help should contact a trusted adult or appropriate emergency resource rather than waiting for Coach Clarke to review a Reflection.

      If information raises a significant safety or welfare concern, CCA may take appropriate human action and may share information as permitted or required by applicable law and CCA safeguarding procedures.

      Parent/Guardian Consent

      By providing my consent and signing below, I acknowledge that:

      * I have read and understand the information above about CCA’s online coaching and athlete-development services, including CCA Progress Studio and Athlete Mental Reflection.
      * I understand that Athlete Mental Reflection is a coaching-development tool and is not medical care, therapy, psychological treatment, diagnosis, or an emergency service.
      * I understand the Reflection privacy boundaries described above, including that parent or guardian access to other CCA information does not automatically provide access to an athlete’s private Reflection content.
      * I understand that information may be shared when reasonably necessary or legally required for athlete safety, welfare, protection, or another lawful purpose.
      * I authorize my athlete to use the CCA online coaching and athlete-development features covered by this consent.
      * I authorize my athlete to participate in Athlete Mental Reflection under the privacy and safety terms described above.
      * I understand that I may contact CCA with questions about this consent or about available options concerning my athlete’s information.

      By signing below, I confirm that I have read and understand this section and consent to my athlete’s participation in CCA’s online coaching and athlete-development services, including CCA Progress Studio and Athlete Mental Reflection.
