I agree to defend, protect, indemnify and hold harmless the Office of Faith Formation, Diocese of Nashville against and from all claims arising from my negligence or fault which arise out of my participation in this activity including but not limited to any personal injury, property damage, disability, death, and sickness or disease including, without limitation, any communicable disease. Additionally, I agree to protect, defend, hold harmless and fully indemnify the Office of Faith Formation, Diocese of Nashville for any claim or cause of action whatsoever arising out of this activity which takes place during the above identified dates that is brought against the Office of Faith Formation, Diocese of Nashville by me or my family members whether such claim arises from the alleged negligence of the Office of Faith Formation, Diocese of Nashville, its employees or agents or my negligence. If any portion of this agreement is held invalid, it is agreed that the balance thereof, shall continue in full legal force and effect.

I agree to abide by any/all policies and rules established for this event. Should I not be able to maintain the guidelines and expectations for this event, I understand there will be consequences for my actions, which could include my being asked to leave the event.

From time to time, pictures and video may be taken. We may use these photographs and videos for promotional and educational material like flyers, brochures, publications, social media, and/or websites. Written consent of the individual or parent/guardian, if applicable, is required. Names will not be posted unless written authorization is given by the individual or and parent/guardian, if applicable, and then only first names will be used.

I authorize and give the irrevocable right and full consent, without limitation or reservation, to the Diocese of Nashville to publish any photograph, video, or testimonial in which I or my child appears. Images may be published in future publications, brochures, flyers, educational materials, webpage, social media, or other promotional materials. There will be no compensation for use of any photograph, video, or testimonial at the time of publication or in the future. I waive the right to approve the final product. I agree that all such portraits, pictures, photographs, video and audio recordings, testimonials, and any reproductions thereof, and all plates, negatives, recording tape and digital files are and shall remain the property of the Diocese of Nashville.

I hereby release, acquit and forever discharge the Diocese of Nashville, their current and former trustees, agents, officers, volunteers, and employees from any and all claims, demands, rights, promises, damages and liabilities arising out of or in connection with the use or distribution of said testimonials, photographs and/or video recordings, including but not limited to any claims for invasion of privacy, appropriation of likeness or defamation.
