      A USI gives you access to your training records and results online.

    • I understand the terms of this Contract and the refund conditions and confirm that I have been fully advised of the fees, refund conditions and conditions of enrolment and agree to be a student at The Unleashed Collective.

      I agree that it is my responsibility to retain a copy of this written agreement as supplied by the RTO, The Unleashed Collective and receipts of any payments of tuition fees or non-tuition fees.

      I agree that under the Data Provision Requirements 2012, The Unleashed Collective is required to collect personal information about me and to disclose that personal information to the National Centre for Vocational Education Research Ltd (NCVER).

      My personal information (including the personal information contained on this application and my training activity data) may be used or disclosed by The Unleashed Collective for statistical, regulatory and research purposes. The Unleashed Collective may disclose my personal information for these purposes to third parties, including:

      • School - if you are a secondary student undertaking VET, including a school-based apprenticeship or traineeship;
      • Employer - if you are enrolled in training paid by your employer;
      • Commonwealth and State or Territory government departments and authorised agencies;
      • NCVER;
      • Organisations conducting student surveys; and
      • Researchers.

      Personal information disclosed to NCVER may be used or disclosed for the following purposes:

      • Issuing a VET Statement of Attainment or VET Qualification, and populating Authenticated VET Transcripts;
      • facilitating statistics and research relating to education, including surveys;
      • understanding how the VET market operates, for policy, workforce planning and consumer information; and
      • administering VET, including program administration, regulation, monitoring and evaluation.

      I may receive an NCVER student survey which may be administered by an NCVER employee, agent or third-party contractor. I may opt out of the survey at the time of being contacted.

      NCVER will collect, hold, use and disclose my personal information in accordance with the Privacy Act 1988 (Cth), the VET Data Policy and all NCVER policies and protocols (including those published on NCVER’s website at www.ncver.edu.au).

      Learner Support Policy: Will be available upon request or if a need is identified by a trainer/assessor. Learning support is offered at pre-enrolment and the pre-enrolment interview will enquire further incl. LLN but not limited.

      For those students becoming a marriage celebrant: The Attorney General's Department applies ongoing fees associated with becoming a marriage celebrant in Australia.       By signing this form, I certify that the information provided is true and correct. I further certify that:

      • I have reviewed the Student Handbook supplied to me and have been informed about my rights and obligations.
      • I have reviewed the Schedule of Fees and Payments and have been informed of the refund policy.
      • I understand that Payment Plans are due on Thursday every week from commencement until paid off, and I will be removed from the program if my plan failed to process on more than 3 occasions (if I selected payment plan and if I am issued with a payment plan).
      • I have reviewed the relevant course brochure and have been informed of the training and assessment services to be provided and the units of competency to be completed.
      • I understand that my well-being is my responsibility but I will inform the provider if I require help with my studies. I have received the student well-being support guide.
      • I have been informed and read factors set by the Attorney Generals Department about becoming a marriage celebrant (if applicable to my course enrolment).

      If you choose a payment plan, payments are deducted weekly on Thursdays from course commencement until the balance is paid in full.

      Eligible students may also qualify for Smart and Skilled supported training for selected courses.

      Please note: If you're studying a CHC30120 Certificate III in Business or CHC32015 Certificate III in Community Services, this training may be fully subsidised by the NSW Government.

      As part of the process, all students complete a short Language, Literacy, Numeracy and Digital Skills (LLND) assessment. This helps us understand any support you may need throughout your studies and ensures the course is the right fit for you.

      The LLND assessment will be emailed to you after you submit this form and can be completed in your own time.
