Waiver & Liability Form
In consideration of my child’s participation in the Eastern Elite Volleyball Summer Camp and related activities, including practices, drills, scrimmages, competitions, travel, and other camp events (“Activities”), I acknowledge and understand that participation in athletic activities involves inherent risks. These risks may include, but are not limited to, bodily injury, serious injury, illness, paralysis, permanent disability, death, and loss of or damage to personal property. I voluntarily and knowingly assume all such risks on behalf of myself and my child.On behalf of myself, my child, and our respective heirs, representatives, and next of kin, I hereby release, waive, discharge, and agree not to sue USA Volleyball, Eastern Elite Volleyball Club, Carolina Region Volleyball, Amateur Athletic Union (AAU), Perry Real Estate, and their respective officers, directors, employees, coaches, volunteers, agents, staff members, and representatives (collectively, the “Released Parties”) from and against any and all claims, liabilities, damages, or causes of action arising out of or related to my child’s participation in the Activities, including claims resulting from the ordinary negligence of the Released Parties, to the fullest extent permitted by law. I certify that my child is physically able to participate in camp activities and has no medical condition that would prevent safe participation. I understand that I am responsible for informing Eastern Elite Volleyball of any medical conditions, allergies, or limitations my child may have. In the event of a medical emergency, I authorize Eastern Elite Volleyball staff and representatives to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that I am responsible for any medical expenses incurred. I further grant Eastern Elite Volleyball the irrevocable right and permission to photograph, film, videotape, and otherwise capture my child’s image, voice, or likeness during camp activities. I authorize Eastern Elite Volleyball to use such materials for promotional, marketing, educational, or other legitimate purposes, including on websites, social media, advertisements, and printed materials, without further notice, consent, or compensation. I understand that this document is a legally binding waiver and release of liability. By signing below, I acknowledge that I have carefully read and fully understand this Agreement and voluntarily agree to its terms on behalf of myself and my child.
