Assumption of Risk and Liability

I, the undersigned parent/legal guardian of the participant listed below, acknowledge and understand that participation in baseball, softball, training sessions, practices, games, camps, conditioning activities, transportation, and all related league events involves inherent risks of physical injury. These risks may include, but are not limited to, falls, collisions, equipment-related injuries, heat-related illness, and other accidental injuries that may occur during participation.

I voluntarily allow my child to participate in the Bayou Parish Youth Sports Developmental League and fully assume all risks associated with participation.

By signing this agreement, I hereby release, waive, discharge, and hold harmless the Bayou Parish Youth Sports Developmental League, its directors, board members, coaches, volunteers, staff, sponsors, facility owners, affiliates, and representatives from any and all claims, liabilities, demands, damages, costs, expenses, or causes of action arising from my child’s participation in league activities, except in cases of gross negligence or intentional misconduct.

I certify that my child is physically able to participate in athletic activities and has no medical condition that would prevent safe participation unless disclosed to the league in writing.

In the event of illness, accident, or emergency, I authorize league officials, coaches, volunteers, or designated medical personnel to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that I am financially responsible for any medical expenses incurred.

I acknowledge and agree to the following player and parent expectations and commitments:

I will encourage good sportsmanship, teamwork, discipline, and respect at all times and will encourage my player(s) to do so as well.

I will support all players, coaches, officials, and league representatives in a positive and respectful manner.

I will not engage, or allow my player(s) to engage in disruptive behavior, harassment, bullying, intimidation, or the use of profanity during league events.

I will allow coaches to coach and will not interfere with practices, games, or coaching decisions.

I understand that player development is the primary focus of the league and that playing time may vary based on attendance, effort, attitude, safety, sportsmanship, skill development, and team needs.

I agree to communicate concerns respectfully and professionally through the appropriate channels.

I understand that failure to follow league policies and expectations may result in disciplinary action, including removal from league activities without refund.

I understand that the league registration fee is NON-REFUNDABLE as we will use these funds to secure venues, purchase equipment, uniforms, etc. 

By signing below, I acknowledge that I have carefully read and fully understand this waiver, release of liability, and participation agreement. I voluntarily agree to its terms on behalf of myself and my child.
