This booking form is for the Cruise and LAND ONLY Components AND the ADD ONS MENTIONED below.  Flights are separate and NOT INCLUDED.

If you would like to ADD FLIGHTS, Please Complete Q's Vacation Request Form and you will receive an invoice within 72 hrs. (choose the "waive fee option" to complete your request) to complete your reservation for booking.

Travel Insurance is separate and NOT INCLUDED: For Travel Insurance request a quote  Agent Max Travel Insurance Quote and you will receive an invoice within 72 hrs to complete your reservation for booking.

Travel Insurance can be added for up 7 days after booking.

Your initial deposit will be deducted from your cruise rate.

This form also serves as an Credit Card Authorization to be placed on file for your room.

This form is to book Only your Cruise Cabin, Hotel Accommodations if selected and ground transportation if selected.

Cruise Cabin must be PAID at the time of booking to secure your reservation.

Monthly payments are required to keep ALL reservations current.(if applicable at time of booking)

Within 5 days after your initial deposit you will receive via email a invoice,  instructions on how to make your monthly payments, monthly payment plan amounts with a link to submit your remaining monthly payments. (if applicable at time of booking).

YOU WILL NEED TO OBTAIN A VALID REAL ID, CERTIFIED BIRTH CERTIFICATE.  *PASSPORT STRONGLY SUGGESTED FOR THE TRIP*. (No Exceptions)

Q's Travel Service will not be liable or owe any traveler any monies for any reservation cancellations due to traveler not having the proper identification or credit cards needed for CRUISE, hotel check-in, flights, excursions, day party or any missed CRUISE PORTS, hotel check-ins, flight departures or any and all no shows to any event.

BY SIGNING THIS DOCUMENT YOU GIVE Q'S TRAVEL SERVICE AND ITS VENDORS PERMISSION TO DEBIT YOUR CREDIT/​ DEBIT CARD FOR THE AMOUNT AGREED UPON ON OR AFTER THE INDICATED DATE.

YOUR SIGNATURE IS REQUIRED TO PROCESS ALL PAYMENTs FOR ALL of YOUR TRAVEL ACCOMMODATIONS and any ADD ONs that you select to SECURE YOUR RESERVATION* 

Reservations must be made in your FULL NAME as it appears on your government-issued REAL ID IDENTIFICATION/DRIVERS LICENSE or Passport.  You MSUT ensure your roommate/s, if applicable, is also making timely monthly payments.

You will be responsible for the full amount due 45 days prior to travel in the event your roommate cancels to keep your reservation active OR travel credits will be made accordingly.

WHAT’S NOT INCLUDED:

• Gratuities for Drivers.
• Flight to West Palm Beach.
• Ground Transfers.
• Travel Insurance must be added 30 Days prior to trip date. 

IMPORTANT TRAVEL INFORMATION:

Provided full payment is received no later than your Final Payment Due date, travel documents, itineraries and travel instructions will be sent via email 30 days or earlier prior to travel.

It is the guests’ responsibility to notify Qs Travel Service of any personal accommodations such as medical, accessible room needs and airport service needs.

Q's Travel Service can add flights to any booking.  OR You MUST purchase your own flight.   

If you purchase your own flight  and you purchased your ground transfers with Q's Travel Service YOU MUST notify us within 30 days prior to departure date via email to ensure your transfers are arranged and confirmed.

General Terms and Conditions

Group Organizer's Responsibilities are:

1. To handle all payments and payment processing.
2. To provide invoices to all guest(s) attending the trip.
3. To provide all guest(s) with updates and balances on payments.
3. To provide all final confirmations and details upon FINAL PAYMENT.

Guest(s) Responsibilities are:

4. To provide neccesary documentation to enter any domestic or International travel.
5. To finance additional items or activities that are not listed.
6. To make all payments according to payment plan agreed on.
7. All guests waive, releases, discharges, and covenants not to sue Q's Travel Service, and affiliates, members, sponsors, organizers, or other representatives or their successors and assigns for injuries or damages of any kind suffered as a result of this vacation or any related activity. At the Hotel/Resort if there are any damages to property or person, the guests are solely responsible to return property to its original state and are responsible for any fees assessed by Hotel/Resort or law enforcement.
8. All payments will be made with Credit/Debit Card. Cash is not acceptable.
9. Guests agrees to full permission, perpetual, and worldwide for organizers to use all names, photographs, video, audio, or quotations in accounts or promotions in any medium.
10. Guests who do not sign this contract will be unable to book this vacation.
11. If payment plan is chosen, guest(s) must pay their deposit to hold their spot.
12. Tour organizers reserves the right to cancel this tour at any time prior to the date of the tour. Refunds will be made in accordance of supplier rules and travel protection rules and guidelines.
13. Making reservations for this package signifies your acceptance of our Terms and Conditions.
14. Reservations must be made in your FULL NAME as it appears on your government REAL ID or Passport. Your name on your invoice MUST match your REAL ID or Passport.
15. Adding a Guest: No Charge
Changing a Guest more than 72 Hours prior to first day sailing: $75.00 Per Booking
Changing a Guest less than 72 Hours prior to first day of sailing: $115.00 Per Person. After final payment date a $135 fee will be imposed to make a name change plus any vendor fees. A invoice will be sent to you via email for payment prior to the name change being completed.

16. Q's Travel Service reserves the right to charge any traveler a $50 cancellation fee and any VIP Experience traveler a $100 cancellation fee to any card you have on file.

20. Hyatt Place West Palm Beach Policy applies for all Hotel Bookings.
