    If selected for a scholarship, you may be asked for additional documentation to support the information you provided in this application. You will also be asked to submit a current photograph for use in IPHA promotional materials.

    The scholarship eligibility requirements are:

    • U.S. citizen and a resident of Illinois
    • Be admitted to or enrolled in a public-health related graduate degree program at a recognized, accredited college or university located in Illinois

    The following items must be included with your scholarship application. Failure to include the required information may affect consideration of your application.
  • A. Student Essay

  • In your own words, provide the following information.  Discuss what factors influenced your choice of public health as a career; your career goals and why you picked the area you wish to pursue after graduation; and, describe how this scholarship would help address your financial needs. Your essay should be no more than 1,000 words.

  • IV. Applicant's E-Signature

  • The decision to award the scholarship will be determined by a committee made up of members of the Illinois Public Health Association and staff. The committee’s decision will be based on a composite evaluation of the student’s application, qualifications and the availability of funds. If awarded, the student must maintain regular enrolled student status during each semester of the award and must not be on scholastic probation, leave of absence, or registered in absentia. Failure to meet these conditions may result in termination of the scholarship. At the discretion of the committee, the scholarship may be reinstated once the student regains regular student status and/or adequately corrects any academic deficiencies.

    Upon selection and verification of enrollment, the Illinois Public Health Association will process the scholarship funds directly to the Financial Aid/Bursar of the recipient's college or university.
