Fashion Ignite Designer Registration ✨🎨

1. General Participation Agreement
By registering for this program, class, event, workshop, showcase, runway show, vendor opportunity, or related activity, I understand that my participation is voluntary and subject to approval, availability, event guidelines, and organizer confirmation.

I agree to follow all rules, instructions, deadlines, schedules, safety guidelines, venue policies, and expectations provided by Peoria Fashion Week Academy, Haus of Runway, Fashion Ignite, Arts Partners, event partners, instructors, coaches, volunteers, staff, and authorized representatives.

I understand that failure to follow expectations, communicate professionally, arrive on time, or comply with safety and conduct requirements may result in removal from the program or event.

Required checkbox:

I have read and agree to the General Participation Agreement.

2. Liability Waiver and Release
I understand that participation in programs, classes, workshops, runway events, fashion shows, vendor events, rehearsals, fittings, photo shoots, performances, travel, setup, breakdown, and related activities may involve risks, including but not limited to personal injury, illness, property damage, lost or stolen items, wardrobe damage, equipment issues, slips, falls, physical movement, use of tools, sewing equipment, styling materials, staging areas, or interaction with other participants and attendees.

To the fullest extent permitted by law, I release, waive, discharge, and hold harmless Peoria Fashion Week Academy, Haus of Runway, Fashion Ignite, Arts Partners, event organizers, instructors, coaches, board members, volunteers, sponsors, contractors, staff, representatives, venue partners, and affiliated organizations from any claims, demands, damages, injuries, losses, liabilities, costs, or expenses arising out of or related to my participation, except where prohibited by law.

3. Assumption of Risk
I understand and accept that participation in public events, creative programming, runway activities, classes, sewing workshops, media production, rehearsals, vendor spaces, showcases, and community events may involve known and unknown risks.

I agree to take reasonable precautions to protect myself, my child if applicable, my garments, belongings, tools, equipment, materials, and personal property.

I understand that I am responsible for determining whether I or my child is physically and emotionally able to participate safely.

I understand and voluntarily assume the risks associated with participation.

4. Media, Photo, Video, and Promotional Release
I grant Peoria Fashion Week Academy, Haus of Runway, Fashion Ignite, Arts Partners, event partners, sponsors, media partners, photographers, videographers, and authorized representatives permission to photograph, film, livestream, record, publish, display, edit, reproduce, and use my name, image, likeness, voice, work, designs, garments, creative materials, participation, and submitted content for promotional, educational, archival, social media, website, press, grant reporting, fundraising, and event-related purposes.

I understand that photos, videos, or recordings may be used without additional compensation. I also understand that media may appear on websites, social media platforms, printed materials, press releases, grant reports, sponsor reports, advertisements, flyers, and future promotional materials.

Required checkbox:

I give permission for my image, likeness, voice, work, and participation to be photographed, recorded, and used for promotional and event-related purposes.

5. Minor Participant Parent/Guardian Consent
For participants under the age of 18:

I certify that I am the parent or legal guardian of the minor participant listed on this form. I give permission for my child to participate in the program, class, workshop, event, rehearsal, showcase, fashion show, photo shoot, or related activity.

I understand the nature of the activity and agree to the participation agreement, liability waiver, assumption of risk, media release, code of conduct, and all other acknowledgments on behalf of myself and my child.

I understand that I am responsible for ensuring my child arrives and is picked up on time, unless other written arrangements have been approved.

7. Code of Conduct
I agree to conduct myself respectfully and professionally at all times. I understand that harassment, bullying, discrimination, threats, unsafe behavior, theft, damage to property, inappropriate language, disruptive conduct, substance use, or failure to follow organizer, instructor, coach, or venue instructions may result in removal from the program or event.

I understand that Peoria Fashion Week Academy and its partners are committed to creating safe, inclusive, creative, and respectful spaces for youth, adults, families, artists, designers, models, vendors, and community members.

I agree to follow the Code of Conduct.

8. Property and Personal Belongings Responsibility
I understand that I am responsible for my own belongings, including but not limited to clothing, garments, accessories, sewing supplies, tools, equipment, phones, bags, vendor materials, displays, personal items, and any items brought by my child, models, assistants, or team members.

I understand that Peoria Fashion Week Academy, Haus of Runway, Fashion Ignite, Arts Partners, event partners, venues, instructors, coaches, volunteers, and staff are not responsible for lost, stolen, damaged, or misplaced property.

I understand that I am responsible for my personal property and belongings.

9. Intellectual Property and Creative Work Acknowledgment
I understand that I retain ownership of my original creative work, designs, garments, artwork, written materials, photography, video, music, branding, logos, and other intellectual property unless a separate written agreement states otherwise.

I grant event organizers permission to photograph, record, describe, promote, display, and share my submitted or presented work for event promotion, documentation, social media, grant reporting, sponsor reporting, archival purposes, and related publicity.

I confirm that any materials I submit or present do not knowingly infringe on the rights of others.
