Travel Fee to travel from Jacksonville, FL to Charlotte, NC on Sunday, May 24 via Bus. (Departs at 3:30am from The View, 9570 Regency Square Blvd, Jacksonville, FL, and will return following our 7pm service back to Jax) -- Card Payments are only accepted; in person payments will not be accepted at the bus. Cost: $50, also includes dinner.

Purpose

This form acknowledges that the participant will be traveling with Kingdom City Church to attend a Sunday service in Charlotte, NC, and releases the church and its leaders from liability during travel.

Agreement

1. I acknowledge that participation in transportation organized by Kingdom City Church involves travel risks including, but not limited to, traffic accidents, injuries, or unforeseen delays.

2. I voluntarily assume all risks associated with travel.

3. I release Kingdom City Church, its staff, pastors, volunteers, and agents from all liability for injuries or losses that may occur during the trip.

4. I agree to follow all rules, guidelines, and directions from church-appointed leaders and transportation staff.

5. I give permission for emergency medical care to be provided if necessary.

6. I understand that failure to adhere to conduct expectations may result in denial of future Kingdom City Church Travel Waiver & Liability Release Form participation.

Note: The Bus will depart at 3:30am on Sunday, May 24 from The View, 9570 Regency Square Blvd, Jacksonville, FL to Charlotte, NC (5.5-Hour Trip)--Please arrive no later than 3:15am. It will return to Jacksonville, FL following the second service (5.5-Hour Trip).
