Pharmacy 45 Appointment Cancellation & Late Policy
To ensure we are able to provide timely and efficient care to all patients, Pharmacy 45 requires the following appointment policies to be observed:
Appointment Cancellations
Patients must provide at least 24 hours notice for any appointment cancellations or rescheduling requests.
Failure to provide proper notice may result in:
A cancellation fee
Limited future scheduling availability
Requirement of prepayment for future appointments
Late Arrival Policy
Patients arriving more than 10–15 minutes late may be required to reschedule their appointment depending on provider availability and scheduled services.
Late arrivals may result in:
Reduced appointment time
Rescheduling of services
Delays affecting other scheduled patients
No-Show Policy
Patients who fail to attend scheduled appointments without notice may be considered a no-show.
Repeated no-shows may result in:
Deposit requirements for future appointments
Limited scheduling privileges
Possible discharge from recurring scheduling services
TSA PreCheck® Appointments
Due to TSA scheduling requirements, missed or late TSA PreCheck appointments may require complete rescheduling based on appointment availability.
Medical Weight Loss & Specialized Services
Certain services may require deposits or consultation fees that are non-refundable if cancellation policies are not followed.
