RescueMeATL Inc. is proud to sponsor your dog's heartworm treatment through one of our veterinary partners. Because heartworm treatment can be a major financial barrier for adopters, we selectively provide sponsorship support to help more dogs leave the shelter system safely and successfully.

This agreement outlines the responsibilities of both RescueMeATL Inc. and the adopter during the treatment process.

Sponsorship Coverage
RescueMeATL Inc. has approved sponsorship of this dog’s heartworm treatment plan through a partner veterinary clinic.

Approved treatment expenses will be paid directly to the veterinary clinic. Sponsorship applies only to the agreed-upon heartworm treatment protocol and does not include unrelated veterinary care, emergency treatment, additional diagnostics, follow-up care outside the approved protocol, or additional medical expenses unless otherwise approved by RescueMeATL Inc.

Any non-approved veterinary expenses remain the responsibility of the adopter.
Veterinary Communication Authorization
The adopter authorizes the veterinary clinic to communicate with RescueMeATL Inc. regarding scheduling, treatment compliance, invoices, and medical updates related to the sponsored heartworm treatment.
Veterinary Care and Liability
The adopter understands that RescueMeATL Inc. is not a veterinary provider and does not supervise or direct medical treatment decisions. All veterinary care and medical recommendations are provided solely by the licensed veterinary clinic.

RescueMeATL Inc., including its volunteers, officers, directors, and representatives, is not responsible for medical complications, treatment outcomes, or additional expenses related to the dog’s care.
Sponsorship Terms
RescueMeATL Inc. reserves the right to discontinue sponsorship assistance if:

Veterinary instructions are not followed
Appointments are repeatedly missed
Communication is lost
The dog is rehomed or surrendered during treatment
The adopter fails to comply with this agreement
Acknowledgment
By signing below, I confirm that I have read and agree to the terms of this Heartworm Treatment Sponsorship Agreement. I understand RescueMeATL Inc. is providing financial support toward approved treatment costs through a partner veterinary clinic, and I agree to participate in the treatment plan as outlined.

I understand that my electronic signature is legally binding and has the same effect as a handwritten signature.
