  • PAYMENT:

    I understand that sessions are $30 for a half hour, $50 for a full hour. These sessions must be purchased in advance either at the front desk or by completing the Payment Form and storing a credit card on file for my trainer to run bi-weekly for sessions completed. I understand these sessions expire 12 months from date of purchase and are non-refundable. I understand I can keep track of these in The Weight Club app, and have been shown where that information is located.

     

    CANCELLATIONS:

    Cancellations should be made at least 24 hours in advance of a scheduled session. Sessions cancelled less than 24 hours in advance will be charged in full to the client.

     

    LATE ARRIVALS:

    Sessions are 30 minutes or 60 minutes, depending on your program. Sessions will not be extended (unless time is available) due to the lateness of the client or due to interruptions caused by the client.

     

    ALL INFORMATION I HAVE GIVEN IS CORRECT:

    All of the information on this form is correct and to the best of my knowledge. I have sought and followed any necessary medical advice and waive my right to obtain a physician's approval. I understand that my trainer has the right to request a physician's approval at any point during my program due to health concerns.

  • INFORMED CONSENT FOR PARTICIPATION IN A PERSONAL TRAINING PROGRAM

     

    • I voluntarily agree to participate in a personal training program, including fitness assessments, exercise activities, and health education related to fitness, nutrition, and stress management. My exercise program will be based on my current fitness level, and I may be required to complete a fitness evaluation before beginning.
    • I understand that I will receive instructions regarding exercise and equipment use, and that a certified personal trainer will supervise and monitor my participation. I agree to follow all instructions and inform staff of any medical conditions, medications, or changes in my health.
    • I understand that I may stop or reduce exercise at any time if I experience symptoms such as dizziness, shortness of breath, pain, or fatigue, and I will immediately notify staff if symptoms occur.
    • I understand that physical contact or positioning may be necessary to ensure proper exercise technique and safety, and I consent to such contact.
    • I acknowledge that exercise carries risks, including muscle or joint injury, abnormal blood pressure, dizziness, heart complications, stroke, or, in rare cases, death. I voluntarily accept these risks and choose to participate.
    • I understand that results are not guaranteed, but participation may improve my fitness, health, and understanding of exercise and equipment use.
    • All personal information obtained through this program will remain confidential and will only be used for fitness and program-related purposes unless otherwise authorized by me or required by law.
    • I have had the opportunity to ask questions and understand that my participation is voluntary.
