Pick-Up Food or Home Delivery

Please select how you will receive the Thanksgiving meal bags.
Please note:

We advise families to pick up their food if they are have some form of transportation. This would also include the ability to ride with another person or the ability to designate a person to pick the food up in your name (they must present the email confirmation and a copy of the recipient's ID).

***Due to delivery being limited, we do advise participants to make every effort to attempt to pick up their food.***

To qualify for home delivery, clients must be:
- over 65 years old,
- disabled,
- and/or no transportation available with no one to pick up bag for you
- residence must be within 10 mile radius of CFBOC.
It is not guaranteed that you will be able to receive delivery.

A CFBOC representative will contact you to confirm you qualify.

I certify that I live within the designated area and meet the income requirements to qualify for this program. I certify that I have submitted a request for one meal bag per household/street address, and I understand that I will be provided one meal bag.
I must bring a valid picture ID that shows my current address. If my address has changed, i must bring both my valid picture ID and a current utility bill with my address.
I realize that if I selected a DELIVERY option, this is not complete until I hear from a representative at CFBOC through email or phone. We will make every effort to contact you.
