    Please provide the title(s) of the photo(s) this consent relates to, so we can match it to the correct entry. The photo title(s) in the model form must match the title(s) in the entry form.

    I confirm that:

    • I am 18 years of age or older.
    • I give Glasgow Harley permission to display the photo(s) I appear in for the following purposes:
      • The People of Motueka online gallery and public voting
      • Physical exhibition at Motueka Library (approximately 2 weeks)
      • The People of Motueka art trail across selected Motueka and wider district venues
      • Permanent display at Glasgow Harley's Motueka office
      • Glasgow Harley’s website, social media, newsletter, and marketing communications related to the competition and Glasgow Harley’s community initiatives
      • Press and media coverage related to the People of Motueka competition and Glasgow Harley's community initiatives.
    • I understand that once a photo is displayed publicly — online or in print — complete removal from all platforms may not always be possible.
    • I can withdraw my consent at any time by contacting Glasgow Harley at info@ghlegal.nz or 03 548 7129. Glasgow Harley will make every reasonable effort to remove the image from future use.
    • Glasgow Harley will store this image securely and will not use it for any purpose beyond those listed above.
    • Glasgow Harley will keep images only for as long as they are needed for the purposes listed above. Some selected works may be displayed for an extended or indefinite period as part of the exhibition, the art trail, or the office display, and those images will be kept for as long as they remain on display. You can ask us to stop using your image at any time, and we will take reasonable steps to do so.
    • I understand this/these photo(s) may be reviewed by an independent judging panel. All judges are bound by a confidentiality agreement and may not copy, save, or share any photos they review.

The permission or licence you give in this form enures for the benefit of that entity.
