  • Welcome to the Application

    Thank you for your interest in participating in this maternal health and family wellness pilot program. This initiative was created to support families through pregnancy, postpartum recovery, and early parenting using a community-centered approach focused on healing, connection, and long-term wellness.

    We understand that many families experience barriers within traditional healthcare systems, including financial stress, fragmented services, limited emotional support, and difficulty accessing consistent care during pregnancy and postpartum. These challenges can become even more difficult for families navigating medically complex pregnancies, infant health conditions, or ongoing caregiving responsibilities.
    This pilot was designed to respond to those realities with compassion, dignity, and practical support.

    Participants in this program will receive access to coordinated prenatal and postpartum support through a collaborative care team that may include doulas, peer support leaders, counselors, wellness practitioners, and healthcare professionals. Families will also have opportunities to participate in community healing spaces, educational workshops, peer gatherings, and supportive experiences that strengthen connection and shared learning.

    In addition to wellness and care coordination services, eligible families may receive flexible monthly financial support intended to reduce stress related to transportation, medical appointments, childcare, food access, or other family stabilization needs. We believe that economic stability plays an important role in maternal and family health outcomes.

    Our goal is to create a safe, supportive, and empowering environment where families feel respected and cared for throughout their journey. Through this pilot, we also hope to strengthen future maternal health systems by demonstrating the value of community-based care models that prioritize trust, relationship-building, and early support.

    What is the Family Wellness Support Pilot?
    The Family Wellness Support Pilot is a family wellness initiative designed to support pregnant and postpartum families through coordinated care, community support, and monthly financial assistance. The pilot recognizes that financial stress can directly impact maternal health, mental wellness, family stability, and a family’s ability to remain engaged in healthcare and support services.

    This pilot was created to provide families with practical support during pregnancy and the first year postpartum while strengthening overall family wellness. The program combines flexible monthly family support funding with community-based services that promote emotional wellness, healthy pregnancies, postpartum recovery, and long-term family stability.

    The Family Wellness Support Pilot brings together doulas, peer support leaders, wellness professionals, counselors, healthcare providers, and community partners to create a collaborative support network for participating families.

    Purpose of the Pilot
    The purpose of the Family Wellness Support Pilot is to:

    Reduce financial stress during pregnancy and postpartum
    Support healthy maternal and infant outcomes
    Increase access to consistent support and wellness services
    Strengthen family stability and emotional wellness
    Improve coordination between families and support systems
    Create a community-centered approach to prenatal and postpartum care
    The pilot recognizes that families caring for medically complex infants or navigating high-risk pregnancies often experience increased medical costs, transportation barriers, missed work opportunities, and emotional stress. By combining wellness support with flexible financial assistance, the pilot seeks to reduce barriers that can negatively impact family health and stability.

    What Will the Family Wellness Support Pilot Provide?
    The Family Wellness Support Pilot will support participating families through the following services and activities:

    Coordinated prenatal and postpartum wellness support
    Connections to community resources and healthcare services
    Monthly wellness gatherings, educational workshops, and peer support opportunities
    Assistance navigating barriers related to transportation, childcare, food access, and healthcare
    Emotional wellness and family support services
    Ongoing communication and support from community-based care providers
    Family-centered wellness planning and stabilization support
    Family Support Funding
    Eligible families participating in the Family Wellness Support Pilot may receive monthly flexible financial support intended to assist with essential family needs, including:

    Transportation to medical appointments
    Childcare support
    Food and household necessities
    Medical-related expenses
    Wellness and recovery support
    Family stabilization needs
    This financial support component is intended to provide families with greater flexibility, stability, and reduced stress during pregnancy and postpartum recovery.

    Pilot Goals
    The Family Wellness Support Pilot aims to:

    Improve maternal wellness and postpartum recovery
    Reduce stressors impacting family health and stability
    Increase participation in prenatal and postpartum support services
    Strengthen community support systems for families
    Improve access to wellness resources and coordinated care
    Support healthier outcomes for parents, infants, and families
    The pilot will also help gather insights on how community-based wellness models and flexible financial support can strengthen maternal and family wellness outcomes over time.

    Sustainability
    The Family Wellness Support Pilot is designed to create long-term impact beyond the initial pilot period. Sustainability efforts will focus on strengthening partnerships, documenting outcomes, and building community-based systems that can continue supporting families over time.

    The pilot will work to establish sustainable relationships with healthcare organizations, community agencies, wellness providers, and local partners to support continued coordination of care and family wellness services. Program data, participant feedback, and wellness outcomes will help inform future funding opportunities, program expansion, and long-term service development.

    Key sustainability strategies include:

    Building strong partnerships with community organizations and healthcare providers
    Training and developing community-based support leaders and doulas
    Creating repeatable wellness and support models that can be expanded in future programs
    Using participant outcomes and feedback to support future grant funding and investment opportunities
    Strengthening referral systems and coordinated care networks

    This program includes participation in the Mental Wellness & Prenatal/Postpartum Health task force. Participants must commit to at least 5 free SOMN wellness activities per month online or in person. Such as online breath work, online journaling, in-person acupuncture, peer support, engagement, Mental Wellness Sessions, Prenatal/Postpartum Education, Peer Support & Community Discussions, Workshops/ Leadership/ Advocacy, or Feedback activities.
