  • Health Information and Consent

    A health officer is on site at all times.

  • Medications, Dosage and Related Conditions

    Please list all prescription and over-the-counter medications the young man is taking. If there are any changes to this list prior to camp, such changes must be communicated to the Stake Young Men Leaders before arrival at camp. All medications must be unexpired and in the original container, and if applicable, with the original pharmacy medication label including the young man's name.
  • Does the Young Man have any allergies?*

  • I understand that by signing electronically I authorize the leaders supervising this event to administer emergency treatment for any accident or illness and to act in my stead in approving necessary medical care. This authorization shall cover this event and travel to and from this event. I assume and shall be responsible for all medical costs and expenses in connection with the treatment contemplated by this waiver.

  • Consent for Participation

    I give permission for my son to participate at young men camp in such activities including, but not limited to, swimming, boating, canoeing, hiking, axe-throwing, Rifle/shotgun shooting, and other standard camp activities at Camp Lee. My son is aware of and agrees to abide by Church standards, the camp dress code, the camp code of conduct, safety rules, and other pertinent instructions. He will abide by Church standards and exemplify Christlike behavior. I understand that participation at young men's camp is not a right but a privilege that can be revoked if my son behaves inappropriately or if he poses a risk to himself or others. Should he be found to have violated any of these conditions by the leaders of the camp, I will be required to pick him up. By signing this electronically, I agree to do so.

  • Young Men Camp Code of Conduct

    The participant agrees to:

    • cheerfully and willingly follow the counsel and direction of all leaders, and abide by the standards of The Church of Jesus Christ of Latter-Day Saints as set forth in the For the Strength of Youth Guide.
    • show respect and kindness to all leaders and young men at camp.
    • be where he is supposed to be and participate in all camp activities.
    • respect the privacy of others and refrain from going through or taking another person's belongings.
    • show respect for camp property and the beauty of nature at camp.
    • wear clothing consistent with church standards as set for in the For the Strenth of Youth Guide.
    • follow the Word of Wisdom
    • use the buddy system
    • respect the sacredness of his body, and others as set forth in affection in the For the Strength of Youth Guide.
    • not gossip or do pranks
    • use his phone responsibly, respectfully, and only at appropriate times.
    • smile and have fun!

  • Camp Dress Code

    While at camp the young man should always wear clothing that is modest and that meets the standards as set forth in the For the Strength of Youth Guide.

    He should be modest at all times, even while in tents and while sleeping.  Swimwear should also be modest. 

  • Media Release 

  • Occassionally, the Fallon Nevada Stake youth leaders would like to post and share photographs, videos, or audio recordings of the youth to our stake social media sites and/or our stake youth website. We need your permission to do so. These posts will be generated and approved by the stake youth leaders.
