I'm so excited you're want to be a part of the Mighty Indeed community! I have spent a great deal of time preparing this program and hope you enjoy it! Before we can start, please fill out this form to register and secure your spot. We're limited to 15 people so make sure you fill everything out to secure your spot! Included below is a sign up form, waiver, and payment for the program :) See you soon!

Waiver & Assumption of Risk

I understand that participation in Mighty Indeed Youth Fitness involves physical activity, including running, jumping, strength exercises, and other movement-based activities. I acknowledge that participation carries inherent risks of injury. I voluntarily allow my child to participate and assume responsibility for any risks associated with doing so. I certify that my child is physically able to participate, and I agree to inform coach Kylie of any relevant medical conditions, injuries, or limitations. In consideration of participation, I release Mighty Indeed and it's representatives from claims arising from ordinary participation in the program.

I grant permission for my child to be photographed and/or filmed during Mighty Indeed Youth Fitness sessions. I understand that these images or videos may be used for promotional purposes, including social media, website content, and marketing materials (I.e. fliers). I understand that no personal identifying information will be shared without additional consent. I release Mighty Indeed and it's respective representatives from any claims related to use of these images or recordings.
Do you give permission for your child to be photographed/video recorded for promotional use? No worries if you say no, you can still participate and we can take group photos, I just won't post ones with your child(ren) it them :)

I'm so grateful for your support and desire to be a part of this program! I care deeply about helping others see their potential and re-write their own limits and truly realize what they're capable of. It's my biggest wish that you enjoy this program, have fun, and mostly importantly, feel PROUD of what you accomplish by the end!
