MEDICATION SHIPPING ACKNOWLEDGMENT
I acknowledge that medications and/or wellness products prescribed or recommended through Bottumzup Health and Wellness, LLC may be dispensed and shipped by a third-party pharmacy, fulfillment center, or vendor.
I understand that certain medications and wellness products, including compounded medications, may be temperature sensitive and should be retrieved, inspected, and properly stored immediately upon delivery according to provided instructions.
I acknowledge that Bottumzup Health and Wellness, LLC coordinates patient care and prescription management; however, shipping, packaging, carrier handling, and delivery logistics are performed through the dispensing pharmacy and shipping carrier.
I understand that Bottumzup Health and Wellness, LLC does not directly control shipping carrier operations, delivery handling, transit conditions, or pharmacy packaging procedures once medication has been released by the dispensing pharmacy and shipping carrier.
I acknowledge that Bottumzup Health and Wellness, LLC will make reasonable efforts to assist in coordinating communication regarding reported shipment concerns when appropriate.
I understand that shipping fees are determined by the dispensing pharmacy and/or shipping carrier and may vary depending on medication type, location, shipping method, carrier rates, and pharmacy policies. Standard shipping fees are non-refundable once medication has been processed and shipped.
I understand that expedited or overnight shipping requests may result in additional shipping charges, which are the responsibility of the patient.
I agree to notify Bottumzup Health and Wellness, LLC promptly regarding any concerns related to shipment condition, packaging, temperature exposure, damaged contents, missing items, or delivery issues upon receipt of medication. I understand that the clinic may coordinate directly with the dispensing pharmacy regarding reported concerns when appropriate.
I understand that replacement requests, refunds, or reshipments are reviewed on a case-by-case basis in coordination with the dispensing pharmacy.
I acknowledge responsibility for:
- Providing an accurate shipping address
- Ensuring timely retrieval of delivered medication
- Proper medication storage after delivery
I understand that once medication has been successfully delivered to the address provided and received, Bottumzup Health and Wellness, LLC is not responsible for delays in retrieval, improper storage, temperature exposure after delivery, mishandling, loss, damage, disposal, or misuse of medication occurring after confirmed delivery.
By signing below, I acknowledge that I have reviewed, read, and understand the above information regarding medication shipment, delivery, storage, and related policies.
