    The seminar covers some of basics of fishing, safety, and an overview of Minnesota DNR regulations along with some hands-on tips from a Pro! Parents are welcome to attend.

  • Fishing is a fun and exciting activity, but it comes with some risks—especially when you're near water. These risks can't always be removed, so it's important to be aware of them. By participating in this activity, I understand that the following dangers and others may exist, including but not limited to:
    1. The risk of drowning while boating, fishing, or wading in the water
    2. Possible injuries from boat parts that move or operate mechanically
    3. The chance of getting hurt while using fishing equipment
    4. Accidental injuries caused by others using fishing equipment nearby
    5. Encounters with wildlife or insects that might lead to injuries
    6. Exposure to the sun, wind, lightning, and other natural elements
    7. The possibility that medical help might not be available right away in case of an emergency.
  • I hereby certify that I am the parent or legal guardian (the "Parent") of the child listed above (the "Child") and that my Child has permission to participate in the NE Minneapolis Lions Club Kids Hooked on Fishing Experience ("Fishing Experience") and be in the care of NE Minneapolis Lions Club members and volunteers during the Fishing Experience.
  • From the time my Child is involved in the Fishing Experience and until my Child is returned to me, the NE Lions Club members will be responsible for the supervision and care of my child.

  • Parent agrees, to waive, release, relinquish and forever discharge the NE Minneapolis Lions Club, all other participants, the hosts, sponsors, partners, volunteers, and all other parties involved in the Fishing Experience (the "Indemnified Parties"), on behalf of himself/herself, and his/her Child, heirs, assigns, personal representatives, and estate, from any and all claims and liability of any sort or kind whatsoever, whether known or unknown, whether in tort or contract, as a consequence or arising out of the Child's participation in the Fishing Experience, and negligent selection, training and supervision of Fishing Experience staff, or negligent supervision or instruction of Child by Fishing Experience Indemnified Parties.

  • I hereby grant the host sponsor the unconditional right to use the name, voice and photographic likeness of the child in connection with any of their audio/video productions, articles, or press releases regarding the Youth Fishing Experience.

  • READ THIS FORM COMPLETELY AND CAREFULLY. BY SIGNING THIS FORM YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF THE INDEMNIFIED PARTIES USE REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE INJURED BY PARTICIPATING IN THIS ACTIVYTY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELMINATED. BY SIGNING THIS FORM YOU ARE GIVING UP YOUR CHILD'S RIGHT AND YOUR RIGHT TO RECOVER FROM THE INDEMNIFIED PARTIES IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM, AND THE INDEMNIFIED PARTIES HAVE THE RIGHT TO REFUSE TO LET YOUR CHILD PARTICIPATE IF YOU DO NOT SIGN THIS FORM.

I realize that I am agreeing to let my child(ren) engage in a potentially dangerous activity. Further, I fully understand the inherent risks of the Fishing Experience and realize that I am giving up my child(ren)'s right, my right, and my spouse's right to recover by lawsuit from the Indemnified Parties for any loss or personal injury, including death, to my child(ren) or any property damage that results from the risks that are a natural part of the activity. I intend my signature to be a complete release of all liability arising from the inherent risks of the Fishing experience to the greatest extent allowed by laws of the State of Minnesota.
